Hi Peter,

I just did my first ivf cycle last week and I am now diagnosed with OHSS.

Here is some background info:

ET was Thursday last week
Fri-started feeling bloated and uncomfortable
Tues- I felt better, clinic told me to take hcg shot to help boost implantation, I took 1 mL
Tues- I felt really bloated had trouble even walking at a snail's pace
Wed night-pain and discomfort so bad I couldn't sleep
Thurs morn- gained almost 4 lbs in one day! saw RE- had lots of fluid, but it was not reaching my lungs, ovaries measured at 6.5
started to drink gatorade and other similiar electrolyte drinks 1.5L/day
Fri-- gained about 1- 1.5 lbs, still feeling very uncomfortable, so bloated now I can move around on my own, a bout of nausea in the evening, very bloated, stomach is now so large I look like I am 5 months PG! From under my breasts all the way down, I am full and bloated ( I am starting to measure the circumference)

So, its Saturday now and I still fell just as bad. Nausea is gone, thank goodness. Occasionally I feel cramps and sharp pains in my lower abdomen area. I can't cough or sneeze without extreme pain. I am also starting to feel discomfort in my upper back, shoulders and neck area.

I am wondering if I should just go to the hospital??

Is the back, neck shoulder pain from lying down or do you think it could be the fluid rising?? I started feeling this yesterday afternoon, and its slowly getting worse.....

Is it better to have bed rest or keep moving around (the house that is)?

Also, some say that OHSS is a good precursor to being PG, is that true???

Any advice is greatly appreciated!! Sorry for all the questions!

i'm by no means a doctor and hopefully Peter will respond...
but your symptoms sound extreme and you should probably at the least go to your ER and get some help! OHSS can become very serious from what i have read.

NGU,

I know that you can get referred pain in the shoulders from abdominal discomfort - fulid pressure on the ovaries and fallopian tubes irritates nerves that signal through the shoulder region. Are you having any trouble breathing? Fluid on the lungs would cause issues with breathing and you would likely feel very heavy in the chest.

I do agree with Lezlie - it might be best to get checked at the ER...or at least an emergency visit to your clinic? They should be in now doing transfers and retrievals for the people cycling over the weekend. Better safe than sorry!

Be careful and sorry to hear you are in such great discomfort...

My guess is that second shot of hCG was what triggered these symptoms. I've never heard of a second shot to help implantation.

Get to the Dr. hun. Don't wait for Peter to respond. Of course his opinion is highly regarded, but OHSS is very serious and you should be seen soon!

NGU- I agree with everyone go to ER. One of the girls from this board was in hospital for over a week for OHSS.

thanks everyone. my breathing still is ok, just can't take deep breathes too well but that might just be cuz i'm so bloated.

i figure if my breathing is ok, do i need to go to the hospital???

HCMM-i think that stupid additional hcg shot did trigger these horrible symptoms. i haven't heard of anyone else doing this either!! i know my clinic is trying to help but what was their logic on this one???

i feel so lucky to have all of you, i read all the posts to dh as he is worried too and we need all the advice we can get!

i am so exhausted since i don't sleep very well with this, so i am going to try and take a nap, see if Peter has had a chance to respond and then decide what to do about the hospital.

NGU - If the pain is that bad, take the advice of your clinic. Drugs you take now will not even cross in to the fetus/embryo as the placenta is not formed. You are best to take care of yourself at this point with the meds, they will not have any effect on the embryo or the pregnancy.

I understand your reluctance to medicate, I have the same issue during the 2ww as I suffer from migraines. But PLEASE take care of yourself first...especially since the meds will not effect the hopeful pregnancy in any negative way.

thanks one day soon- i guess we all get so worried about taking meds during 2ww. i am going to hold off still on the tylenol, call me stubborn,but i want to wait until monday which is my next b/w to see where my betas numbers are. my real beta is on thurs feb 7th.

dana- oh geez, i just reread your post about peter not replying wkends....... oh dear! i don't know what to do ....... i am so indecisive about going to the hospital, just the thought of sitting there for possible for hours makes my stomach hurt more! you see sitting is very uncomfortable for me with all this bloatedness so that's partly why i don't want to go to hospital unless i have to.....

Did you try calling your clinic? One of the weekend REs might be in and be able to see you....just a thought? It might save a needless ER visit? I am worried about you!

My advice- Call your clinic. The will more than likely have an on Call RE that will call you back. Most Emergency rooms have no clue what OHSS is. I had severe OHSS last cycle and it was in my lungs and it was terrible. However I was told by my RE that a regular emergency room would be a mistake. The best thing to do is make the call. Take your tylenol and gravol if you need to and rest. Make sure you get up on the hour to walk for a tiny bit. That is what I was told. I have mild OHSS this time and am taking the percocet at night with the gravol to sleep. Your ovaries need the rest. Get adequate fluids and eat very small meals. I remember the pressue is unbelievable. You will get through it though! I am surprised that they gave you another shot of HCG. I believe the increase in HCG (which happens when you are pregnant) exacerbates OHSS so the shot is probably what made you sicker.

Take care.......... and don't worry about the tylenol 3's. I worried like you and the clinic reassured me over and over that it is safe. Advil or ibruprofen is bad for the fetus.

I was taking Tylenol every 6 hrs NGU for my migraine... Me RE kept on telling me there was no problem at all with them... You need to rest and take care of yourself !!!

onedaysoon- i saw my clinic thurs and fri. i could call them, but i don't think they will tell me anything different at this point.

zoey- this is interesting and good to know about emergency rooms. my clinic is closed this weekend, so they advised me to go to the hospital if i felt worse. i will get up every hr though as you suggested. good advice. did you have to drink water and gatorade?? i can barely fit anymore liquids in me.

jozerr- thanks for reassurences. i would prefer if there was something to drain the fluid more than anything else.....

what did it feel like when you were getting fluid in your lungs, can you tell before it gets really bad?

NGU - take care of yourself. If you feel a little worse, call your RE again. You can never be too careful. Take the tylenol. My clinic told me I could take tylenol for headaches, etc.

thanks bal, its just a slow constant throb. i have also been getting cramps :crsd: but i barely notice it with the ohss.

i'm just taking it real easy today, i just want to make it till monday morning fornext u/s and b/w.

Hi there,

My sister and I had the same problem. I got really ill and had to be on an IV and my sister had to have fluid removed. Mine was triggered by the shot, my sister by pregnancy. After throwing up all night, I phoned my RE and he had me come to the clinic where he could closely monitor me and put me on IV. Maybe I missed that post, but if you have not done so, I would phone your RE. Good luck!

NGU,

How are you doing today? I have been thinking of you...I hope your symptoms have subsided a bit! Let us know if you have a chance, I am sure I am not the only one concerned for you!

Good luck tomorrow with your beta and your appointment with your RE, hopefully they will be able to make things more comfortable for you :D .

i have been in touch with my RE, saw them thurs and fri, will see them tommorrow. it is so awful isn't this? i think this is really the worse thing from cycling.

i am no better and no worse today. thanks for asking. i went to bed by 9pm last nite, my body is wiped! they say if i am pg, it will get worse or take longer to go away, so i hope this pain lasts! :sigh: i have gained about 9 lbs in 4-5 days now. it is really awful.

Just checking in. Yes, it can get worse or take longer to get rid of if you are pregnant. I did get pregnant and I did feel like crud for a while, but I am now 14 weeks and it is all a distant memory! Good luck!

I KNOIW and I FEEL YOUR PAIN. I was in the hospital with severe OHSS for a week. I gained over 30lbs of fluid within one week. They couldn't even drain the fluid from me because my ovaries were so enlarged (over 25cm) and they were concerned that they would puncture an ovary. The only thing that helped was the morphine they kept pumping me with. I ended up pregnant and the symptoms lingered for quite some time after. I hope you start feeling better soon. Get as much rest as possible (I know it is hard because it is so uncomfortable to even lie down).

OMG, how long did your ohss last???

I have been doing more b/w than usual since my clinic is checking my fluid and electrolytes. I am PG!!! But my real official beta is actually tommorrow. So, I will feel much better when I find out numbers tommorrow.

I ahve gained about 11 lbs now in one week. My body is just so sore with the weight and bloatedness.

Is ohss dangerous when PG??

I was hoping it was good news for you! It's not good that the symptoms are worse, but I'm so happy you are pregnant and you should be fine with OHSS. Sounds like you are getting some good monitoring!

i am thinking of asking re if i could i am in sooo much pain and its not subsiding at all.

how long did your ohss last??

I didn't have mine drained. They told me to drink 50/50 Gatorade and water. The Gatorade balances the salt in your cells and will help you eliminate more.

My symptoms lasted 1 week but they weren't as severe as you, as far as I can tell. You'll get back to normal, just keep drinking!!
