Blighted Ovum, clinic letting me down

I found out Oct 5 that it's a blighted ovum.   I was 8w4d then.   I was told to stop progesterone and estrogen and wait a week.

 

A week passed and I started bleeding, but I still haven't miscarried.  I have already missed a week and a day of work.

 

My clinic doesn't want to do a D&C.   They are giving me the impression that they are too overbooked at their hospital to do it and to wait or start cytotec.

 

My husband died in Feb 2013, 2 days after his TESA and the night after my egg retrieval.  I am so scared something could happen to me when I am alone in the night.  I have a 6 year old that went through hell when his daddy died.  I am almost out of sick time, I lost most after DH died and just started to build it back up.

 

I have been in touch with the clinic every day and am waiting for today's call.  It's been 3 hours since I left the message.   I am feeling like they are pushing me to my breaking point, so I will just do the cytotec.

 

My questions are:

1.  What should I expect from my clinic right now?  The have made tens of thousands of dollars from me.

2.  How long can I be bleeding without major cramps?  I'm on day 4.

3.  How do I keep sane?


Becky (35)  Unexpectedly widowed in Feb 2013.  The night after my ER.  I was married to an incredible man named Chris.

Son, age 6
TTC #2 (his first) since our wedding in July 2011.

Dec 1, 2011- Found out he has azoospermia (no sperm in SA.)  Totally heartbroken and shocked.
Jan 2012- First consult at fertility clinic.
March 2012- My tests show high LH but everything else was normal. His LH/FSH were high, testosterone was really low. (4.2)
May 17, 2012- Official Dx of testicular failure and non-obstructive azoo. Given the option of mTESE, TESA or using a donor.
June 7, 2012- Urologist is seeing us again per our request. We need him to answer some questions about the procedures and testosterone.  He would not consider Clomid or any hormones for DH.
June 27, 2012- Saw RE and decided to proceed with dIUI. Told we needed one cycle on the BCP because of my high LH.
June 28, 2012- Started BCP for 21 days. Did pre-IUI micro and blood testing on July 3rd. Told they should be in on time for IUI in early August.
July 21, 2012- CD1. Called clinic and found out my cycle was canceled. They messed up and my file wasn't ready. They forgot to draw vit D and some other results are missing.
August 9, 2012- Nurse left message saying they cannot do this cycle either. Now it will take 2-3 MONTHS for DH's blood tests before we can use donor sperm.
August 10, 2012- RE called. DH has a reactive Hepatitis C blood test and they are trying to figure out if he's got an active infection or was exposed in the past.
August 23, 2012- He's Hep C NEGATIVE! Phew!
October 2012- Latest SA results still shows azoo, but we're glad the testosterone looks normal now. dIUI after 21 days on the pill. Clomid for cd3-7, LH is too high (17 and 14.6) and progestone getting too high. Had 3, 13.5mm folliciles and my lining is 8.6. CD13- Lining was good, triple layer at about 9. Now I just have one 17.5mm follicle on my left side.
October 17, 2012- dIUI  canceled. Clomid didn't work, estrogen too low, LH was normal again and progesterone was off too. Now we need an appointment to talk to our RE again to start over.
Dec 2012- Saw RE and we have to do IVF/ICSI, even with a donor. So, we decided to throw in a TESA with donor backup. Bloodwork this cycle, BCP and mock in Jan and hopefully a Feb IVF/ICSI/TESA!
 
IVF #1
Mock done, teaching visit done.  Started Suprefact on Jan 30.  AF arrived on schedule.
Baseline done, starting Gonal-F and Luveris Feb 11.
Stim day 4-  1 follicle at 11mm, 40 more under 10.  My lining is already a triple lining and at 8.6
Stim day 6-  8-9 follices between 11-13mm at least 20 more under 10mm.  This doctor saw my lining at 6.3?   Encouraged to walk because my circulation to my uterus wasn't what they wanted to be this time.  No idea what that means.
Stim day 8-   14 follicles.  All measuring between 11 and 15 with more under 10.   Lining is over 10.  Circulation is good again.   They are letting me skip my day 9 appointment. 
Stim day 10-  TRIGGER tonight!   I have 14 between 15-21mm, a few more at 14 that may catch up.  Lining is over 10.   DH's TESA is tomorrow.
Feb 21-  No sperm found with TESA.   ER tomorrow with donor sperm.
Feb 22-  ER was successful!  Collected 18 eggs.   7 from my right and 8 on my left.  We can skip ICSI because the donor's sperm is great.  Crossing fingers for a good fert report tomorrow.
June 2013- Updating. My husband, Chris, passed away unexpectedly of Sudden Arrhythmia Death Syndrome just hours after my ER on Feb 23.  Had 14 fertilize.  9, 5-day frozen blasts are at the clinic waiting for me to make some decisions. 

 

FET #1

June 2014-  5-day blast.   BFN just in time for what would have been our 3rd anniversary.  

 

FET#2

August 2014.  FET #2 on Aug 25 with another 5-day blast.  BFP!   Had scare with a bleed at 4w5d, but beta is still doubling.  Due May 13, 2015.  Ultrasound on Oct 5.

Oct 5-  8w4d-  Blighted Ovum.  Stopping estrogen and progesterone.  Waiting to miscarry.  Looking forward to moving on with life.

Oct 17- D&C.   Feeling peace about being at the end of the infertility battle.  Looking forward to the next chapter in my life.

I just spoke to the nurse and she gave me some tough love. She pretty much told me I was just being impatient and she left notes for the doctors and I need to come in and tell the doctors how I feel. She told me to come whether I am bleeding or not. I asked her if there's a point where I shouldn't drive by myself they one hour to get there and she told me I'm a tough cookie and to do it anyway. So, I go in tomorrow morning at 7:30 AM and I don't even know if it's going to help me. I was doing so well emotionally when this all started and now I feel like I am ready to lose it

MapleBecky, I understand your frustration and fear but as long as it is not an ectopic pregnancy, risks are minimal (from what I was told with mine, at 7weeks and three days. I let mine happen naturally, without medicaton or medical intervention. Ultimately it is up to you but I know most clinics prefer you pass it naturally. Good luck

I just worry being along when/if it happens.   I had my husband die in the middle of the night in our bathroom, and it scares me to think of myself being alone in there if I start feeling weak.

How long did you have to wait?   I don't know how much sick time I can spend on this, I am soon to be out sick without pay.

MB - Do you have a GP you can call?  I think it's time you spoke with someone else.  Or, time to head to the ER and get a referral there.  I REALLY do not think you should do cytotec alone.  Even the manufacturers do not recommend going it on your own.  You absolutely need someone there to make sure you are ok.  

 

Please.  See another dr.  I really think it is time.  I am so incredibly sorry that you are going through this.

Head to your local emergency room with a trusted friend or family member. Don't bother with your clinic if you are not feeling supported. Trust your instincts and realize that your fears are valid because of everything you have been through and you deserve care from practitioners who respect you.

MapleBecky...I'm so sorry you are going through this. After all you've been through, you deserve more from your clinic! I can't believe they are saying you are being impatient! I think you just want some reasonable answers as to what to expect and how long it could take. You paid them an insane amount of money, the least they could have done is have you see the doctor to discuss all 3 options (watchful waiting, cytotec, D&C) and their side effects, risks, benefits. They should also have discussed the miscarriage process and what to expect. It sounds like you are seeing the MD tomorrow but if they brush you off, please do see your family doctor! They can counsel you on what to expect and on all the management options. If they don't know then I'm certain they can refer you to an early pregnancy loss clinic or an OB/GYN ASAP! You deserve piece of mind!  Just because you want to get the show on the road, does not make you impatient! You have been patient enough.

 

I hope the miscarriage process is quick and as painless as possible. I'm be thinking about you!

Summary:

Secondary infertility due to Mosaic Triple X syndrome, endometriosis and teratospermia.

 

IVF#1 (Sept 2013) with fresh transfer: BFN.

IVF#2 (March 2014) converted to IUI due to poor response: BFN

IVF#2.1 (October 2014): freeze-all due to PGS. 2 euploid blastocysts, 2 "no results".

FET#1 (December 2014): chemical pregnancy.

FET#2 (March 2015): BFN. No more euploid blasts. sad.png

FET#3 (Nov 2015): after many cancelled cycles we finally transferred our last 2 "no result" blasts. hcg 329 at 9pd5dt th_abfp.gif

3.  How do I keep sane?

Realizing of course that our situations are different, all I can offer you is results of my experience.

 

1. My clinic was unable to help me as I had already been 'released' from the fertility program.  My referral to the Early Pregnancy Loss clinic had to be sent from my GP (though later proved unnecessary).  I'm not sure how your clinic works regarding this, but your GP is always able to help with this stuff, and often times, easier to get a hold of I assume?

 

2.  At my 13W+0D ultrasound, it was measuring 5 days behind at 12W+1D.  At 15W+6D ultrasound, it showed growth stopped 2 days after the 13W+0D ultrasound.  I didn't have the miscarriage for 4 more days, therefore it took a little over 3 weeks for me to miscarry after growth stopped.  My miscarriage was apparently similar to a labour, and all bleeding and cramps were on the day of the miscarriage only. Edited to add: Of course I kept bleeding later, like an extremely heavy period for quite awhile, but it was manageable.

 

3.  I don't know how to tell you to keep sane.  By all rights you shouldn't be able to, but there's this thing in us that keeps yelling 'IS THAT ALL YOU'VE GOT' at the universe...well, when we aren't damning it for the way it's been treating us to that point.  I'm sorry you're going through this.  Being on the board proved very helpful for me, even if you go into radio silence for awhile.  It helped me feel like I was part of this group of women who had seen it all, that collectively there was nothing I should feel alone on.

I am so sorry you are going through hell and back, you really have had a horrible year and this should not be happening.  How insensitive can this clinic be?   One of my miscarriages took forever to end, I went to the ER for the same concerns as you are having, plus I have severe anemia and the constant bleeding was not good. They had the on call OB do a D&C and I was in and out in 12 hours.  It was the best decision. I suggest you not wait and just go to the ER. If anything they can administer meds to progress the MC and control the pain that comes along with it.

Hi Becky
I've had 2 m/c's, one in the ER with Cytotec, and one at home. It's pretty awful either way. The Cytotec one was much further along, and was a mini labour. The second was at 6.5 weeks. I took mega doses of vitamin C, primrose oil & black cohosh as I was told the OB GYN's were both on holidays. A D&C seems simpler, but there is a downside of uterine scarring. I'm not sure what the difference in deciding b/w letting it occur naturally or a D&C, but the one with Cytotec was much quicker than the one at home.
Try the ER & call your GP for a referral. It's really an awful, sad experience, I'm so sorry.
TTC - off/on since Jan 2009
Diagnosis - Male factor - Undescended testicle at birth, cancer of same testicle in 2005.
Most recent SA: 46 million/mL, 55% motility, 2% morphology (ouch)
Worst SA: 10 dead sperm (March 2011)

2 miscarriages - Sept. 2012 (trisomy 22) & Sept. 2013
Hashimoto's diagnosis - AntiTPO of 1147!

50mcg Synthyroid and feeling 1000000x better

FSH=7
AMH=3.3
AFC=22
Karotype normal

Hoping for a good cycle and lots of good embryos to freeze (not optimistic about the first round).

I have an update. I saw a different doctor at the clinic today and she was very compassionate. She booked me for her next or day which will be Friday morning.

I had tried to call my family doctor yesterday and the earliest appointment they could give me was next Tuesday. I miss my old family doctor, he just retired in the spring. The new guy works a lot in emergency and his family practice comes second.

I went to emerg last week for my double ear infection in the doctor there told me to see my family doctor. Since I was coming from a private clinic from a different hospital, he didn't feel that the gynecologists would be in a hurry to help me.

I'm crossing my fingers that my body holds out until Friday and I can wake up knowing it's all done.

TTC #2 (his first) since our wedding in July 2011.

Dec 1, 2011- Found out he has azoospermia (no sperm in SA.)  Totally heartbroken and shocked.
Jan 2012- First consult at fertility clinic.
March 2012- My tests show high LH but everything else was normal. His LH/FSH were high, testosterone was really low. (4.2)
May 17, 2012- Official Dx of testicular failure and non-obstructive azoo. Given the option of mTESE, TESA or using a donor.
June 7, 2012- Urologist is seeing us again per our request. We need him to answer some questions about the procedures and testosterone.  He would not consider Clomid or any hormones for DH.
June 27, 2012- Saw RE and decided to proceed with dIUI. Told we needed one cycle on the BCP because of my high LH.
June 28, 2012- Started BCP for 21 days. Did pre-IUI micro and blood testing on July 3rd. Told they should be in on time for IUI in early August.
July 21, 2012- CD1. Called clinic and found out my cycle was canceled. They messed up and my file wasn't ready. They forgot to draw vit D and some other results are missing.
August 9, 2012- Nurse left message saying they cannot do this cycle either. Now it will take 2-3 MONTHS for DH's blood tests before we can use donor sperm.
August 10, 2012- RE called. DH has a reactive Hepatitis C blood test and they are trying to figure out if he's got an active infection or was exposed in the past.
August 23, 2012- He's Hep C NEGATIVE! Phew!
October 2012- Latest SA results still shows azoo, but we're glad the testosterone looks normal now. dIUI after 21 days on the pill. Clomid for cd3-7, LH is too high (17 and 14.6) and progestone getting too high. Had 3, 13.5mm folliciles and my lining is 8.6. CD13- Lining was good, triple layer at about 9. Now I just have one 17.5mm follicle on my left side.
October 17, 2012- dIUI  canceled. Clomid didn't work, estrogen too low, LH was normal again and progesterone was off too. Now we need an appointment to talk to our RE again to start over.
Dec 2012- Saw RE and we have to do IVF/ICSI, even with a donor. So, we decided to throw in a TESA with donor backup. Bloodwork this cycle, BCP and mock in Jan and hopefully a Feb IVF/ICSI/TESA!
 
IVF #1
Mock done, teaching visit done.  Started Suprefact on Jan 30.  AF arrived on schedule.
Baseline done, starting Gonal-F and Luveris Feb 11.
Stim day 4-  1 follicle at 11mm, 40 more under 10.  My lining is already a triple lining and at 8.6
Stim day 6-  8-9 follices between 11-13mm at least 20 more under 10mm.  This doctor saw my lining at 6.3?   Encouraged to walk because my circulation to my uterus wasn't what they wanted to be this time.  No idea what that means.
Stim day 8-   14 follicles.  All measuring between 11 and 15 with more under 10.   Lining is over 10.  Circulation is good again.   They are letting me skip my day 9 appointment. 
Stim day 10-  TRIGGER tonight!   I have 14 between 15-21mm, a few more at 14 that may catch up.  Lining is over 10.   DH's TESA is tomorrow.
Feb 21-  No sperm found with TESA.   ER tomorrow with donor sperm.
Feb 22-  ER was successful!  Collected 18 eggs.   7 from my right and 8 on my left.  We can skip ICSI because the donor's sperm is great.  Crossing fingers for a good fert report tomorrow.
June 2013- Updating. My husband, Chris, passed away unexpectedly of Sudden Arrhythmia Death Syndrome just hours after my ER on Feb 23.  Had 14 fertilize.  9, 5-day frozen blasts are at the clinic waiting for me to make some decisions. 

 

FET #1

June 2014-  5-day blast.   BFN just in time for what would have been our 3rd anniversary.  

 

FET#2

August 2014.  FET #2 on Aug 25 with another 5-day blast.  BFP!   Had scare with a bleed at 4w5d, but beta is still doubling.  Due May 13, 2015.  Ultrasound on Oct 5.

Oct 5-  8w4d-  Blighted Ovum.  Stopping estrogen and progesterone.  Waiting to miscarry.  Looking forward to moving on with life.

Oct 17- D&C.   Feeling peace about being at the end of the infertility battle.  Looking forward to the next chapter in my life.

MapleBecky - would you qualify for short term disability? I just went through surgery for an ectopic pregnancy last week. My work put me on disability which had a 7 day waiting period (ate up all my sick time for this year). I don't know the full specs yet of my plan, but it will give you some leverage to miss more time off work. 

 

After experiencing my loss, I am so disappointed in the lack of resources and support for women. Experiencing a miscarriage is stressful enough. Your clinic should not be giving you the run around on top of it.

August 2012 - Start TTC naturally.

 

October 2013 - Appointment with OB/GYN. HSG, blood, SA tests, all results normal. dx Unexplained. 

November 2013 - Natural cycle monitoring. BFN

December 2013 - 1st round Clomid. Ovulated early with timed intercourse. IUI cancelled. BFN

January 2014 - IUI #1 - BFN. 

February 2014 - IUI #2 - BFN.

May 2014 - IUI #3 -  BFN. 

August 2014 - IVF + ISCI. BFP!

Oct 7, 2014 - 7 weeks, 1 day - Ectopic rupture. Lost left tube & our Angel Baby 8388.gif

 

January 2015 - FET. BFP! Beta #1 - 55. Beta #2 - 278. Beta #3 - 709!!!

Saw our beautiful baby on the ultrasound measuring perfectly with a strong heartbeat.

EDD October 15, 2015. 

 

Wyatt arrived 9 days overdue on October 24, 2015 weighing 9 lbs and measuring 21 inches long. He's perfect! We are so grateful for our baby boy. heart.gif

I don't think short term disability works for this.   If I get sick again, I may need to look into it.     I should be ready to go back to work on Monday.

 

C&P for my other post:

 

I had my D&C today and feel so relieved.  I wasn't even so sedated that I couldn't talk throughout the 10 minute procedure.   Recovery was a bit tougher, cramps and moderate bleeding, but it settled within an hour an a half.   I went to mom's and slept and feel great tonight.  Still bleeding a bit, but I am no longer terrified of something happening in the middle of the night.

 

This is the end of my IVF/TTC days.   I will pop in from time to time, but I really feel like this chapter in my life is coming to a close.    I gave my 2 best embryos a chance and it didn't work out.   I guess they weren't meant to work out.    I feel peaceful about knowing I tried and now I feel ready to live in the present and stop looking behind me.

Just a quick update.  I am 1 week post op.  Still spotting dark stuff but I feel so good now.  It took a few days.   They aren't doing any betas and I am done with the clinic.  The worst was the YI I got from all the antibiotics I had and all the wiping I did when I was bleeding before the miscarriage.

 

I am surprisingly doing fine about the loss.  After losing a spouse, this is so different.  I feel like nature spared me of more hardship. 

 

I spoke with the clinic one last time today and that chapter is pretty much closed.  I just will sign the consent to donate my remaining blasts to the clinic in the spring.   I really feel good about being done.  I have been through enough.

I've been moved and inspired by you and how you have moved through things. 

I can feel the palpable sense of relief to move on with your life and leave all this infertility stuff behind. Bravo woman. Bravo.
