I need some advise as to where to go from here. My beta was yesterday and it was confirmed another chemical pregnancies (7th documented one plus many more undocumented) because it came back at only 21 @ 11dp5dt. It is being redrawn to make sure it's gone down to zero (which I think it will because I started getting positive hpt's on 5dp5dt on sticks that measured 20-25mIU). This was with a PGS normal embryo so no issues with it plus we did intrilipids to try to combat nk cells (no where in Canada tests for nk cells & I can't afford another $3000-5500 to send to a lab in the States). I have unexplained infertility and now recurrent loss. Nothing shows on any test that we have done. Everything comes back clear. I still have 4 frozen PGS normal embryos but I don't want to use them up to have it happen again. With the protocol we did it was a 6% chance of another loss and we fell into it. Where do I go from here?? What tests do I need to push for??

June 2012-Started taking Estrace for estrogen priming protocol
Blood test to see why af hasn't started. BFP with low numbers. Chemical Pregnancy
IVF#1 (KRF/Genesis)--July 29, 2012- Estrogen Priming Protocol
August 19-Egg Retrieval 20 eggs; 18 fertilized; 11 made it to Day 5
August 24- Transferred 1 Blast; Froze 4 (the rest didn't make it overnight to Day 6)
Sept 2-Took HPT (Faint Positive)
Sept 5-Beta 3 (BFN--Another Chemical)
FET#1(KRF/Genesis)-Oct 30 2012 On Estrace and Prednisone. Transferred 2 blasts (Had to thaw 3-one didn't make it. One left in the freezer)
Nov 13 2012- Beta- BFN

FET#2(KRF/Genesis) -Nov 16,2013 BETA Nov 27--Chemical Pregnancy

waitingforbfp - I am so sorry that this has happened again. I'm trying to recall but I think I was told by the RE once that even a PGS normal embryo can for whatever reason stop developing. Just because it is chromosomally normal doesn't mean it will be viable. There have been women on this forum who have had all abnormal results and they still transferred it resulting in a positive healthy baby. Mind you, I think this is a case by case basis. Don't give up on your other little embryo's. One of them could be your baby. I know its hard and I've been there myself. Just my non-medical opinion. Have they done a laparoscopy to look for endometriosis or other potential causes? Again, I'm very sorry ((hugs))

Thanks. Yep...I've had a few laps and nothing was found. I am going to push for another one though. Has to be a reason for so many losses all about the same time. Just don't want to use our last embryos and have it happen again and again. This was our last retrieval due to finances. And now I found out my clinic is raising it's prices again (third time since I've been with them).

Have you done testing for clotting disorders and other autoimmude disorders?

 

Sorry this keeps happening to you. I hope you get your little miracle from the 4 embryos you have let. ((HUGS))

I read an article through this link (can't remember which one, but it's the one where she's a diplomat's wife) where her body couldn't absorb folic acid so she kept having early miscarriages.

Me - now 36 Him - 37
Trying to conceive since our wedding day May 25, 2008.
Started Clomid Jan-April 2012.
Referred to RFP June 2012.
First consult at RFP November 2012.
First IVF cycle on antagonist protocol (GonalF, Luveris) January 2013.
Transferred two day-3 embryos, had severe OHSS, lost one twin between 7-13 weeks but was expecting one little one October 2013.
Found out at 6 mos that our little one had died weeks earlier and had to induce and deliver stillborn, July 25, 2013.
Something found on adrenal gland at follow up ultrasound, MRI showed what seems to be a benign tumor. Meeting with specialist Jan 8, 2014 - all fertility treatments on hold. :/
Tumour is definitely producing Aldosterone (causes high blood pressure) and Cortisol. Lucky me, only 34 similar cases recorded in the world! Tumour removed May 2014.
Approached about a possible adoption March 2014. Rush through all the paperwork and process.
Charlotte Evangeline born June 27, 2014. We brought her home on July 7 and held our breath for 10 days until she officially became ours.
Surprise BFP October 2014. Due to wonky cycles, ultrasounds convince Doctors I have a blighted ovum but detailed ultrasound reveals heartbeat at 6w3d. On progesterone support and waiting to see if pregnancy is still viable. Holding our breath again.
Adalyn Marie born on June 9, 2014 after a roller coaster pregnancy.

SGD--I have done the clotting tests and the immune panel (that my clinic offers anyhow). I want everything redone, I'm thinking. Can't hurt to check it all again. xx

Personally, if you definitely think another retrieval is out of the question, then I'd probably think about a gestational carrier - preferably a woman who has successfully carried before.  I'd just be too scared about risking the last 4 embryos. 

 

Good luck - it really sucks to have done the PGS and think you're on your way to get such a let down.

First let me say how sorry I am that you are going through this. I feel your pain because I have had multiple miscarriages and nothing is ever found. Like you, I have tested for everything under the sun. I also pushed for karyotyping and that came back clean too. I know you did intralipids this time around, but for me since I am currently cycling, my clinic has put me on prednisone which is a steroid. This cycle is different from the rest because they added that plus the intralipids which I will also be doing. I am not sure if you tried the prednisone but it may be worth asking about it, although I really don't know if it has any effect as of yet. Also, how are your progesterone levels, they also supplement me with endometrium 3x a day with the oil. I hope you find some answers.

Unexplained infertility is an awful diagnosis, we expect science to have answers.
I'm guessing you've done the karotype testing? Sperm DNA fragment test? I know in canada they aren't big on NK test and intralipid.
What was the protocol? Steroids? Aspirin? Viagra suppositories?

It's so frustrating to deal with infertility, then the physical/emotional/financial cost of treatment. If you need to talk in real life, I'm in Rossland and around this weekend.

Hoping for a good cycle and lots of good embryos to freeze (not optimistic about the first round).

I also wanted to add, have you had your thyroid and antibodies checked. This was an issue that went undetected for me. My naturopath actually found it and then my clinic confirmed it. I am currently on 30mg of desiccated thyroid. Apparently thyroid also plays a role in recurrent miscarriages. I found an abundance of information of American pregnancy society on thyroidism and its role on miscarriage. Good luck and keep me posted.

Thanks amazing grace. Yep...had my thyroid tested and all normal there. Have a new plan for next FET (biopsy, blood thinners, intrillipids, doxycycline, prednisone, oral vs vaginally progesterone? & and a few other things). Hope it helps this round.

I know how frustrating it can be especially when there are no answers. I am in the same boat. They will be adding prednisone and intralipid this time around. I am doing my intralipid next Friday and I am a little nervous. Let me know how things go. Do you have a date yet?

Just my 2 cents of advise..Do you think it's time to swtich RE for second opinion? I have been reading Dr.Sher's blog under open forum. I have read cases like yours and got their bfp with this Dr.  I lost one too and I know how it feels. I will keep in you in my prayers. I highly recommend browsing Dr. Sher website and you can even ask a question under open forum and he reply promptly.

 

Your case could under autoimmune or alloimmune.

June 2014: Will start bcp on long lupron protocol

June 2014: Cancelled IVF due to clinic timing with my ER..plan to cyle first week of September as per clinic

July 14, 2014: Returned to RE to asked if there is way I can cycle this month? Plan to manipulate my period with clinic timing for my Egg Retrieval... feeling hot in the evening grabbed my internet cheapie pregnancy test and lo and behold 2 lines appeared within seconds...not faint but very obvious line..did not believed maybe it was evap!

 

July 15, 2014: Tested with FMU and still got 2 lines..this time I am hopeful..called my clinic and got my beta hcg it's 97 @10dpo 206 @ 12dpo 2328 @17dpo and 5800 @ 19dpo

 

August 6, 2014 : First ultrasound saw our little miracle with a hb of 145...so in love thought 6w5d but baby is measuring 7w1d.

I m praying for a healthy pregnancy. I believed in miracle!

 

August 15, 2014: 2nd ultrasound, 8w3d baby is measuring 8w6d head started to develop with a hb 179....so happy and relieved.

 

Sept,04 2014: First IPS screening, baby is looking like a baby now:) jumping up and down and waving his/her hands. All measurements is looking good...with a hb 171.Thank you God!

 

Sept 22, 2014: 4th ultrasound. Baby is active with a hb of 149. Normal growth. Yay!

 

Oct. 27, 2014: 5th ultrasound/Anatomy scan (19w2d) It's a GIRL! Looking great and baby is active! Normal anatomy scan! Thank God!

 

Due date: March 24, 2015

 

Arabella arrived on her due date March 24 at 1012pm weighing 7 lbs and 12 0z, and 21 inches tall. My family is complete! Thank you God.

Maybe a consult with Dr. Sher? It's usually a few hundred bucks for them to review everything and give a second opinion but it might be worth it.

I am doing the endo scratch and intrillipids on the 19th (Thursday). I have done both before. Intrillipids are easy just takes forever. Bring a book.

Thanks Soontobemommy. I read Dr Sher's blogs and watch the webinars constantly (that's how I came up with a protocol for this FET). I did a skype consult wih him also. Amazing man & if I had unlimited money, I would go to Vegas to be treated by him. He is NOT cheap. Unfortunately, not many clinics in Canada test for these issues (mine being one of them), but at least they are willing to do the treatments without the testing. We will not be going in for another retrieval, so these 4 PGS normal embryos are all we have. Thought about moving them also, but the cost and hassle to get them across the border is too much stress for me to deal with. I'd hate to get held up at the border and have them all go to waste.

 

Maybe a consult with Dr. Sher? It's usually a few hundred bucks for them to review everything and give a second opinion but it might be worth it.

I did a consult with him. His suggestion was to come there for testing. Only $5000 for basic testing :/ Not to mention the flights, hotels etc. We are already about $50000 in debt from treatments, so don't want to go in deeper than we are.

OMG! I'm just using curse words in my head at the cost of testing & infertility in general. I'm so sorry, it's just plain awful to have no answers, and not even knowing what questions to ask.

I have also read Dr. Sher's blogs and am very familiar with his program. I too have been tested for everything under the sun but nothing comes back. Most of the tests recommended by Dr. Sher I have had. I actually use him as a resource. If I am missing something, please let me know.

Have you had an HSG? Not a sonogram of the womb but a hysterosalpingogram, which looks at your womb and your fallopian tubes. The test involves putting a dye solution in there and one of the things they check is whether the dye gets all the way up the tubes or not. An HSG can show whether you have any sort of blockage in either tube, and it's important because if you do there may be fluid at the site of the blockage, and if that fluid leaks into your womb (as it typically does), it can end an early pregnancy.

 

Sometimes the HSG alone is both diagnosis and treatment, because minor blockages can be swept away by the dye solution. If the blockage is more substantial you may need other treatment. But it's very important to know whether you've got any sort of tubal blockage. My clinic (CCRM) will not do an embryo transfer unless you've had a recent HSG.

It is being redrawn to make sure it's gone down to zero (which I think it will because I started getting positive hpt's on 5dp5dt on sticks that measured 20-25mIU). This was with a PGS normal embryo so no issues with it plus we did intrilipids to try to combat nk cells (no where in Canada tests for nk cells & I can't afford another $3000-5500 to send to a lab in the States). I have unexplained infertility and now recurrent loss. Nothing shows on any test that we have done. Everything comes back clear. I still have 4 frozen PGS normal embryos but I don't want to use them up to have it happen again. With the protocol we did it was a 6% chance of another loss and we fell into it. Where do I go from here?? What tests do I need to push for??

Have you had an HSG? Not a sonogram of the womb but a hysterosalpingogram, which looks at your womb and your fallopian tubes. The test involves putting a dye solution in there and one of the things they check is whether the dye gets all the way up the tubes or not. An HSG can show whether you have any sort of blockage in either tube, and it's important because if you do there may be fluid at the site of the blockage, and if that fluid leaks into your womb (as it typically does), it can end an early pregnancy.

 

Sometimes the HSG alone is both diagnosis and treatment, because minor blockages can be swept away by the dye solution. If the blockage is more substantial you may need other treatment. But it's very important to know whether you've got any sort of tubal blockage. My clinic (CCRM) will not do an embryo transfer unless you've had a recent HSG.

 

Thanks Ideagirl. Yep...we have done multiple hsg's. We have been trying now for over 14 years so that was one of the first tests we did and had redone many times. Not too sure how accurate that test is though. In all my times doing it, we got multiple different readings (right tube blocked left open, left tube blocked right open, both tubes open, both tubes closed). We finally did one when I was under a sedation having a lap and both were open (which they both are). That's when they told me 7/10 peoples tubes spasm when the test is done (reason for the so called blockages). I recommend everyone doing it now under sedation so there is no false blockages.

Man, that's a rough road you've been on. I'm so sorry.

 

When was the last time you did an HSG? CCRM requires that they be done within a year before transfer, since things can change.

I did my last one within a year (I think it was after our first failed fresh cycle way back when). They are now calling it an obsolete test since it can give so many spasms in the tubes. They now prefer the sonohyterogram because it doesn't cause as much cramping and they can get in where they need with the camera.

Hi waitingforbfp,

 

I know this was years ago but were you ever able to carry to term? I'm in a similar boat with two chemicals with tested embryos (I'm pretty sure i'm having a second miscarriage as I type this).

This thread was filled with valuable insight.

I'm sure I'm missing something obvious here, but if you have a transfer and the embryo is placed in the uterus, why are sonos and hysteroscopy tests still relevant? (Years in the IF world and I'm still confused!)
