Cycling at MFC from Ottawa

Hi ladies, we decided to try for a second baby. Given my history at the OFC, I am switching clinics. So I will start cycling at the Montreal Fertility Clinic. I have an appointment next week to go over the details. I know some ladies have done this before. Any tips/ recommendations will be greatly appreciated (monitoring cycling at OFC, travel, blood tests, etc)

Nov - Dec 09: Baseline ultrasound, blood test etc etc.. Low antral follicle count, one blocked tube and ovarian cyst
March 10: Lap for cyst, ended up having endo stage 4, endo in bowel, endometrioma, and two blocked tubes
May-June 10: IVF # 1 - antagonist protocol, 2 transferred (6 cell grade 3), no frosties, BFN
Nov-Dec 10 : IVF # 2 - antagonist protocol + ICSI, 2 transferred (8 and 9 cell grade 3), no frosties, BFN....sigh....

Hi,

I am currently cycling at MFC. I know some opted to be monitored at OFC. OFC does an excellent satelite monitoring for a fee I hear. It usually takes 6 appointments for a short protocol- first consultation, three us then retrieval then transfer. I usually takes the earliest appt possible (around 6:45) so that I can get into work around 9:30. They no longer can give you drugs on site so expect to go to the pharmacy on the day of first consultation and your last us. I usually take metro (say takes about 30 mins then 5 min walk from the station). I park on Vendome street (2 hrs allowed most days from 8 am to 4 PM maybe). Just one thing that I want to bring to your attention is that even if you pay for the treatment (if you are Ontario resident), I think the clinic is still governed by the Quebec law in terms of how many embryos can be transferred. I know somebody else from this forum went to T.O. instead because of that restriction. I would ensure you ask that to Dr. M. They do not take your blood at every appt like in OFC. The nurse took the blood at the last us. That was it I think. You will see the difference.  Most of the times (except this time I cycled as the REs were at a conference), it is the REs who do us. If you want to find out more about satelite monitoring at OFC, I know Sandi replied to someone else in the past. I believe it was a couple of hundreds. If there is anything, let me know. Good luck.

July 10 - Consultation at OFC
All test results normal (did not do Kruger test or AMH test) and was told to try for another 6 months
FSH around 6, antral foli count 9 (on the left)

Dec 10 - Asked to be transferred to MTL (OVO) from OFC
April 11 - Saw Dr. Jamal, antral folli count 8 (on the left) and AMH test show low ovarain reserve (due to a single ovary). Given Clomid for two cycles, had bad reaction (very cranky) and BFN. Dr. Jamal suggested Natural IVF without stimulation. Hated OVO dearly (long wait time, extreme lack of organization, nurses, etc). Googled and found MFC. Consultation booked in Sept. 11

Sept. 11 - Saw Dr. Sophia at MFC. She suggests a standard IVF.
Sept. 11 - Learned that private insurance thru work have coverage up to 20K for IVF (Thank goodness!)
Decided to go for 1st trial at OFC given proximity.

IVF #1 (Oct 11) at OFC (Dr. Haebe) - Puregon 400 IU for 9 days. 9 leading follicles (and 8 smaller ones), which resulted in 5 eggs (but one degenerated). Only 2 fertilized thru ICSI (embryo quality second best and average). Day 3 transfer of 2 embryos, which resulted in BFN

IVF #2 (Nov 11) at MFC - Puregon 300 IU and Menopure 75 IU for 10 days. 8 leading follicles (and some smaller ones). egg collection Nov. 16/11. 7 eggs collected out of 8 follies. 6 are mature and ALL SIX FERTILIZED!!! 5 day transfer of 1 blast Nov. 21/11. Beta scheduled Dec. 2nd 1st Beta 10dp5dt 287. 2nd beta scheduled Dec. 5th 2nd Beta 14dp5dt 1,524

1st U/S Dec. 19th (6wks & 6 days): heartbeat measured at 137bpm, baby measuring 7.2mm, cervix measured at 32.9mm

 

August 12, 2012: A beautiful baby girl born at 11:21 pm measuring 8 lbs 3 ounces/53 cm long

Stimulated for 8 days trigger April 8. ER April 10. Out of 8 follicles, 6 eggs retrieved. Only 1 egg was mature & fertilized. Clinic recommends Day 2 transfer. We declined Day 2 transfer given this may be a complete waste.

Dr.M. understood our concern & agreed. Booked for a Day 5 transfer (but will be a miracle if this one embryo makes it to Day 5). Dr. M. advised that the embryo on Day 2 is 4 cell grade 1. So there is little hope.

Ok no call until Tuesday, which means we are going in for a transfer on Day 5. ET on April 15. Transferred an excellent quality blastocyst. Now 2WW. Blood test April 28. BFN

 

2.3.

CD1 August 18. First consult on August 19. Spoke with Dr. M about two wishes - minimum 10 day stimulation & 5 day transfer. Stimulated for 11 days. ER September 4. Out of 9 follicles, 6 eggs, 4 mature and all fertilized. Dr. M called and suggest double Day 3 transfer. However, DH veto a double transfer. Sticking to Day 5 transfer. ET September 9. Suprisingly out of 4 embryos on Day 3, 3 made to blastocysts. Transfer 1 Day 5 blastocyst. Shocking call the next day saying two left over embryos were good enough for freezing. 2ww Turned out to be a chemical pregnancy

Thanks Anais for your response. To be honest, I don't want to deal with OFC. I think I will go to Montreal for each US and procedure. I asked about the regulations and given my age I will get 2 embryos transferred. In previous IVFs, I never got more than 3 fertilized anyway. Where did you buy your meds? In Ottawa? Do you take the train or drive?

Hi,

I drive to MTL and buy meds in MTL. I live in Aylmer so the law says that I need to purchase  drugs in QC to have it covered although I have private coverage. If you are doing a short protocol, it is not that bad. It is total 6 visits and I make my husband drive me for the last two. Tomorrow is our ET. I am hoping that we get no calls tomorrow morning telling us none made to Day 5. Best of luck.

By the way, I don't blame you for not wanting to deal with OFC. After my failed cycle at OFC, I went to MFC right away and delayed my WTF appt with Dr. Haebe. When I saw him for WTF appt, he blamed my age (I was 35 and 7 week pregnant). He I guess just could not accept that their protocol was no good. I was not too upset because I still have it covered by a private plan. Imagine you paid out of your pocket some bloody $12K...

When I cycled at MFC, I did everything there - US, drugs, etc.  At that time (before free IVF), I would get appts at 10am or so and drive down in the morning and come home same day..the only time I stayed in MTL was night before ER because it was less stress and my ER was at 8:30am.  Even for the transfer, I drove in the morning of, as it was scheduled at noon.  Dr. M gives late appt if he knows you are coming from out of town...again this was before free IVF, so I don't know if things changed.

Now with that said, I did my most recent IVF in Toronto and did all my monitoring at OFC.  They are GREAT at monitoring...not the IVF part, but I had no complaints about monitoring with them.  But it's costly $250 for consent signing and about $200 per appt for US and bloodwork, my total bill was $1500.  But less expensive then going to TO.

 

You can now get all your drugs from OFC - their pharmacy is now seperate, so get a prescription from Dr. M and Barbara will be able to fill it without having to get OFC to approve it or anything else.  Much easier than getting it in Montreal.

Hi,

Ok I just had an ET today. I will share with you my schedule for my past cycle.

I had an initial appt (CD3) on Wed. I was told to start stimulating from Friday. So 4 days of stimulation, I had an us on Tuesday. Then again on Friday. They told me to start on orgalutron on Friday. Went back on Monday for another us. Had a retrieval on Wed. Then transfer on Monday. So in total 6 appts. I take the earliest appt possible 6:45. Left home at 3:30 (call me crazy). The appt only takes 10 mins then drive back to work. On a day ER, usually this is in the morning like at 9:45 (then you need to be there at 8:45). We left home at 5 (even brought our 14 month old with us). Expect a delay for sure like an hour. I was so upset because for ER, they tell you to do a trigger shot at precise time.  I didn't know what happens after 36 hours from the trigger shot. They were running 1 hour late. I did not like the nurse lecturing me how everyone is waiting. I just wanted to know what happens after 36 hours. If I knew, I would not have become that anxious. So most women ovulate between 38 to 42 hours so even if they are late, you are ok. Dr. M. came in to see me before ER and told me. For ET, we had it at noon today. Another one hour delay. My bladder was about to pop. Expect waiting. They are quick but just manage your expectation. You still get excellent service. They manage to get us a blastocyst from 2 embryos that fertilized. I am very happy. Best of luck!!!!!

Out of 9 follicles, 6 eggs, 4 mature and all fertilized. Dr. M called and suggest double Day 3 transfer. However, DH veto a double transfer. Sticking to Day 5 transfer. ET September 9. Suprisingly out of 4 embryos on Day 3, 3 made to blastocysts. Transfer 1 Day 5 blastocyst. Shocking call the next day saying two left over embryos were good enough for freezing. 2ww Turned out to be a chemical pregnancy

Anais - sending you huge sticky vibes... Dr. M does miracles and hoping one of those 2 is it for you!!! You are crazy... getting up at 3:30 with a 14 mth old :)  but whatever works... I just took the day off from work the day I went up to do monitoring, less stress :)

 

Amelia is almost 7 months old and it has been hell since she has been born.  What I didn't know until she was 5 months old is that she is allergic to milk and soy.  I've had the fussiest, most difficult baby since birth - since day 1.  Everyone said it was colic and to suck it up.  I knew it was something else, but no one believed me....not even my friends.. it seemed she was ok when others were around.  Anyways finally saw the gastro at Cheo who diagnosed her at 5 months...by this point she has been pooing green for months now, was loosing weight and had stopped nursing (went from every 2-3 hours to every 4-5 hours and longer) She was in constant pain and was now associating it with my breastfeeding her.  So i'm now dairy and soy free for 2 months and only now are we seeing improvement.  SHe just started solids a few days ago and it seems to be ok... we tried cereals at 6 months and it was a disaster... it takes 6 weeks for dairy and soy to be out of my system and another 2 weeks after to be out of hers...she doesn't seem to be in pain anymore, but has developed the worse sleep habits ever...i have been sleep deprived for 7 months now...so we are trying to make it better for her.  I just feel so bad for her, she's been in so much pain for a while and no one listened or believed me...so frustrating... anyways we are finally on a road to recovery and I hope to start enjoying my mat leave soon as I only have 5 months left... but i'm thankful for her and that overall she is healthy - some others are not so lucky.  She should outgrow this by the age of 2.  We tried special formula, but she won't take it..so i'll be on this diet for a while.. i was originally planning on weaning her around 1 year old, but I think i'll go back to work and continue to nurse her until she outgrows this, so she can get the nutrients she needs.  How is your little one?

Oh poor mom and poor Amelia. It is frustrating when they have allergies and they have no ways of telling other than crying.

I am glad at least you figure that out now so you know what you are dealing with. Anais also has a couple of allergies including to peaches which can not be confirmed by all means - skin test, blood work, etc. Maybe you think you can start the solids sooner so she can get all the nutrients that she needs that way? Some babies are great with solids - not mine until almost a year. Now she is a pig. I call her piglet, a bottemless one. Hope things get better soon. I brougth Anais before a year mark to see an allergist. I think we saw him when she was 10 months. I was freaking out of possible another anaphylaxis. We did a skin test at 10  months. We now know she is allergic to wheat, eggs, possibly soy too (he said 50% likelihood). So it has been no wheat, no egg diet but it is ok. We are very hesitant to try new food. So I can understand and sympathize with you. Other that, she is great. Quite funny already. I am on dreadful 2WW although I feel something is cooking inside!

FSH around 6, antral foli count 9 (on the left)

Dec 10 - Asked to be transferred to MTL (OVO) from OFC
April 11 - Saw Dr. Jamal, antral folli count 8 (on the left) and AMH test show low ovarain reserve (due to a single ovary). Given Clomid for two cycles, had bad reaction (very cranky) and BFN. Dr. Jamal suggested Natural IVF without stimulation. Hated OVO dearly (long wait time, extreme lack of organization, nurses, etc). Googled and found MFC. Consultation booked in Sept. 11

Sept. 11 - Saw Dr. Sophia at MFC. She suggests a standard IVF.
Sept. 11 - Learned that private insurance thru work have coverage up to 20K for IVF (Thank goodness!)
Decided to go for 1st trial at OFC given proximity.

IVF #1 (Oct 11) at OFC (Dr. Haebe) - Puregon 400 IU for 9 days. 9 leading follicles (and 8 smaller ones), which resulted in 5 eggs (but one degenerated). Only 2 fertilized thru ICSI (embryo quality second best and average). Day 3 transfer of 2 embryos, which resulted in BFN

IVF #2 (Nov 11) at MFC - Puregon 300 IU and Menopure 75 IU for 10 days. 8 leading follicles (and some smaller ones). egg collection Nov. 16/11. 7 eggs collected out of 8 follies. 6 are mature and ALL SIX FERTILIZED!!! 5 day transfer of 1 blast Nov. 21/11. Beta scheduled Dec. 2nd 1st Beta 10dp5dt 287. 2nd beta scheduled Dec. 5th 2nd Beta 14dp5dt 1,524

Dr.M. understood our concern & agreed. Booked for a Day 5 transfer (but will be a miracle if this one embryo makes it to Day 5). Dr. M. advised that the embryo on Day 2 is 4 cell grade 1. So there is little hope.

Thanks for the info! We decided to do everything at the Montreal clinic. That means going back and forth which is OK. I agree with other people that the clinic seems to be saturated. I waited over a hour for the first appointment, over an hour for the first ultrasound and 1 hour 30 min for the first ultrasound for IVF. I have a pretty flexible schedule so I can wait, but it might not be worth it to do all the travelling otherwise. Dr M seems to be very caring and patient. I bought my meds at the OFC pharmacy (now called Green Valley pharmacy).
