Hi,

My DH and I are about to embark upon our 2nd IUI, but Dr. M spoke to us today about possibly moving on to IVF for our next cycle if the IUI doesn't work.

He said the entire procedure is only about 5-10 minutes, and that I'd be sedated. He also made it sound like it would be "uncomfortable" but that I wouldn't really be aware of what was going on.

It's a lot to come at us at once (we've only done the 1 IUI), so we have a lot of thinking to do, but was just wondering—how bad/painful is the entire procedure—from taking the meds and beyond?

Thanks!

Me: 32, DH: 31

Everyone experience is different depending on personal pain tolerance and how much sedation the clinic administer, but I can say the pain during the retrieval is the smallest tiniest most irrelevant part of the IVF process, imho.
If you have done even a single IUI with injectable medicine, you know exactly what is the stimulation part of the IVF is involved. The difference is the dose of the meds, and maybe the length of the stimulation. As far as physical pain goes, the dose does n't matter. Where it is really matters is the financial department . The higher dose/longer stimulation means much higher cost on meds, not to mention the IVF cost itself.
The egg transfer is exactly like the insemination during the IUI, except they do not place sperm into your uterus, but fluid with embryos floating in it. So really the only difference is the ER, which is like 10 minutes procedure. If you are really afraid of the pain, you always can pay for general anaesthesia, for an extra $500 or so. Personally I think that is just a waste of money.

However because it is much more at stake, the process is zillion times more stressful than an insemination. The 2ww is torture with IUI, but the stress is nothing compared to the stress after IVF. Plus there is this time between retrieval and transfer when your embryos are growing in a petri dish and you are so freaked out of the fact if they will make it to the transfer or not.
I think the emotional burden of the IVF is much larger not just because much more $$$ and chance is involved, but because there is no further steps. When you do an IUI, you always have the back- up plan for failure. You know if it is doesn't work out, you can go further and do an IVF. When you do an IVF, you know there is nothing more to do. If it fails , especially if it fails multiple times, you know that you will not be able to be pregnant with a bio kid ever. This is just the most incredible stress you can take.

Because I'm in the States, my IVF retrieval were all done w/ concious sedation (asleep mostly, but not a general) and I felt zero pain. The first IVF was done w/ just a pain med (oral), and that was significant pain -- I ate crackers at 6 am for a 10 am retrieval....oops. It all depends on your pain tolerance - for me it wasn't bad.

The shots get old.... because of the bruising (no matter what you do to prevent them) -- but it's not pain.

Good luck!

Did they tell you why you need IVF after only one IUI?

I agree with Ope, the pain of possibly never having a child, makes the discomfort of the egg retrieval and injections seem like nothing. But maybe that's just our opinion :) I got IV Fentanyl, and it lasted about 10 mins, and it was 'uncomfortable' at times. I would do it a million times over if I needed to in order to be where I am now, pregnant.

Ope - Thanks for sharing your experience, that makes a lot of sense. I hadn't even thought of the stress of worrying about the embryos making it :S

Vetter - I think that would be a similar experience to what ours would be (the dr. said a combo of a narcotic with Versed). Were you not supposed to eat before the procedure? Did that make it more painful? We're going to do a 2nd IUI, but I guess because my egg follicles seem to range from being more "modest" (4-6 a cycle pre-maturing) to a good amount (10), I think the dr is assuming the next month's cycle will produce a larger stock to work with, and he doesn't want to miss the opportunity.

Berry - Thanks for your input! That is completely true when you look at it that way. And congrats on your pregnancy!

I can say that the ET was so painful and I was VERY aware of what was going on in the room and down there!!!
It felt like a wee pin picking at berries in a bush... I can't describe it any other way!

I have a tattoo and that was nothing like the ET!!

I have an overly long cervix (I am 5'0 so I am tall somewhere - too bad no one sees it!!) The ET was more annoying for me because of the long cervix and the bend in it to get to my uterus. I am now PUPO!!!! I am just in that long 2WW. I am hoping for a postive BETA on the 25th :)

Good luck and all the best to you guys!

Fentanyl & versed is conscious sedation. I honestly remember very little of the procedure & was only mildly uncomfortable at the end. If its anything different than that - you have the right to ask for more. Explain that you're uncomfortable & I'm sure they won't argue with you.

Hoping your iui works & all of this is just noise!

Hi there,
I personally never had IUI but if your IUI was with injectable drugs, it would be pretty much the same with IVF. I had two IVFs done and we got pregnant at the 2nd try. Both times, retrieval was done with mild sedation and almost felt nothing. I think it is more of mental stress than physical. I can say holding not to pee for the procedure was more painful than the procedure itself.
Because a lot more is at stake, mentally it is more stressful but you can get support from this forum. One thing to remember is your chance of having a baby is a lot higher with IVF. I have the most beautiful 9 week girl in front of me (snoring) and I would do it again any time for another child. Good luck and it will happen!

TTC since Dec. 09
Me (35) him (30)
Unexplained infertility but have only one ovary (lost right to dermoid cyst in Nov. 07) and he has low morphology (5%)

July 10 - Consultation at OFC
All test results normal (did not do Kruger test or AMH test) and was told to try for another 6 months
FSH around 6, antral foli count 9 (on the left)

Dec 10 - Asked to be transferred to MTL (OVO) from OFC
April 11 - Saw Dr. Jamal, antral folli count 8 (on the left) and AMH test show low ovarain reserve (due to a single ovary). Given Clomid for two cycles, had bad reaction (very cranky) and BFN. Dr. Jamal suggested Natural IVF without stimulation. Hated OVO dearly (long wait time, extreme lack of organization, nurses, etc). Googled and found MFC. Consultation booked in Sept. 11

Sept. 11 - Saw Dr. Sophia at MFC. She suggests a standard IVF.
Sept. 11 - Learned that private insurance thru work have coverage up to 20K for IVF (Thank goodness!)
Decided to go for 1st trial at OFC given proximity.

IVF #1 (Oct 11) at OFC (Dr. Haebe) - Puregon 400 IU for 9 days. 9 leading follicles (and 8 smaller ones), which resulted in 5 eggs (but one degenerated). Only 2 fertilized thru ICSI (embryo quality second best and average). Day 3 transfer of 2 embryos, which resulted in BFN

IVF #2 (Nov 11) at MFC - Puregon 300 IU and Menopure 75 IU for 10 days. 8 leading follicles (and some smaller ones). egg collection Nov. 16/11. 7 eggs collected out of 8 follies. 6 are mature and ALL SIX FERTILIZED!!! 5 day transfer of 1 blast Nov. 21/11. Beta scheduled Dec. 2nd 1st Beta 10dp5dt 287. 2nd beta scheduled Dec. 5th 2nd Beta 14dp5dt 1,524

1st U/S Dec. 19th (6wks & 6 days): heartbeat measured at 137bpm, baby measuring 7.2mm, cervix measured at 32.9mm

August 12, 2012: A beautiful baby girl born at 11:21 pm measuring 8 lbs 3 ounces/53 cm long

Trying for Number 2

October 7, 2013 - CD1

October 9, 2013 - First consultation

Start stimulation Friday October 11 (Puregon 300 & Menopur 75) for 10 days

Last us on October 21, 2013 7 leading follicles - Trigger shot tonight (Ovidral)

Retrieval October 23, 2013 - 6 eggs retrieved

Ferti report October 24, 2013 - 4 mature eggs and only 2 fertilized

At this point, I am losing hope.

Oct 25, 2013 - Surprisingly Dr. M. wants us in for 5 day transfer

ET October 28, 2013: one young blastocyst to transfer

Viability scan shows fetus behind schedule by a couple of days

Repeat U/S

Discovered on Dec 10/2013 that fetus stopped growing at 7wk4days (Missed miscarriage)

2.2.

CD1 March 29. First consult on March 31. Same protocol.

Stimulated for 8 days trigger April 8. ER April 10. Out of 8 follicles, 6 eggs retrieved. Only 1 egg was mature & fertilized. Clinic recommends Day 2 transfer. We declined Day 2 transfer given this may be a complete waste.

Dr.M. understood our concern & agreed. Booked for a Day 5 transfer (but will be a miracle if this one embryo makes it to Day 5). Dr. M. advised that the embryo on Day 2 is 4 cell grade 1. So there is little hope.

Ok no call until Tuesday, which means we are going in for a transfer on Day 5. ET on April 15. Transferred an excellent quality blastocyst. Now 2WW. Blood test April 28. BFN

2.3.

CD1 August 18. First consult on August 19. Spoke with Dr. M about two wishes - minimum 10 day stimulation & 5 day transfer. Stimulated for 11 days. ER September 4. Out of 9 follicles, 6 eggs, 4 mature and all fertilized. Dr. M called and suggest double Day 3 transfer. However, DH veto a double transfer. Sticking to Day 5 transfer. ET September 9. Suprisingly out of 4 embryos on Day 3, 3 made to blastocysts. Transfer 1 Day 5 blastocyst. Shocking call the next day saying two left over embryos were good enough for freezing. 2ww Turned out to be a chemical pregnancy

2.3.1.

FET CD1 Sept 27 First consultation Sept 30 Oct 16 Transferred two blastocysts 2ww

Started nausea 3dp5dt it can`t be this early POAS 5dp5dt readable BFP! Wow

Summer - I did all my IVF cycles with Dr. M at MFC. Unless they changed what they do for sedation, you are conscious the procedure with painkillers being administered through an IV drip. I did not feel much pain for my first retrieval and recovered relatively easily. My last cycle however was very painful. Really it comes down to your own pain tolerance. They will also prescribe you some anti-anxiety meds that you can take the night before retrieval to help you sleep and not feel so anxious for the procedure. Up to you whether you take it, but it helped me immensely. That said, I wish you luck with you IUI and that all this discussion will be moot as you won't need to resort to it :)

Hello!

My ER was not painful at all! A little bit of sting, that was all.

I asked to nurse to give me a meds as much as she can because I am weak for pain,
and she said she cannot give too much because I will feel bad after. But she tried to give me more.

After 17 follicles collected, I had not much pain!

You can ask the dose of meds if you are scared:)
