Hello Dr. Hollands,

Was just wondering what your thoughts were on Two day Vs. Three day transfer of embryos.

Is it not better to wait for the three day? What are the chances of achieveing pregancy with a two day blast?

I look forward to hearing from you

Thank you,
August

I have always done a day 3 transfer but now my RE wants to do a day 5 because he feels it predicts better embryos .... so if day 5's are "better" then day 3's then I would assume day 3's are "better" then day 2's - but honestly I dont know the correct answer.

I don't think one is necessarily better than the other but the further along the embryo grows the better you can "select" a good embryo to transfer. You could have a beatiful looking embryo on day 3 but it arrests on day 4 but a not so good looking one will go in to day 5. I think it has more to do with picking the most viable embryo and the best way to do that is to see which ones make it to 5 day blast. That being said, there is also a school of thought that the embryo does better inside the body than outside so getting them back quickly is the norm. I only had 2 on day and did not feel comfortable going to day 5 in case they both arrested and then I would have nothing to transfer. I had previously transferred (3) 3 day embryos and (2) 5 day frozen blasts, all were unsuccessful.
Me (Kerri-Lyn) 41, DH (Steve) 37, TTC since April 2007Unexplained IF (possibly crappy eggs, AMH 10.4 = poor responder) - HSG, SHGx3, Lap, Cycle Monitoring, Recurrent loss bloods, EMB, Karyotyping, DNA Frag - all normal! 7 IUI's, 2 cancelled IVF's, 3 complete IVF's, 1 FET, 2 clinics, 1 early miscarriage, lots of debt, lots of heartache, lots of tears. We rolled the dice and got lucky on our last ditch "close the door" cycle and have a beautiful little girl. April 2014 - Had the crazy notion to try for another. Shocked with a BFP, miscarried @ 8W6D. See my "about me" page for more cycle details.

If the number of embryos that you have to choose from exceeds what you are willing to transfer, going longer may make sense as more info exists on their quality the longer you go.

As for me, I just had 7 eggs retrieved (don't know fert. yet) and I can transfer up to 6 on day 3. Or 4 blasts on day 5. I wish someone could tell me which way is better to go. My RE will lean towards putting whatever is left back in on day 3, but would let me choose if I prefer. If I had a guarantee that a good egg couldn't die off before day 5 because it didn't like the culture, I'd go to day 5 so that more of the aneploid embryos could die off first...and hopefully make a miscarriage less likely. And I wouldn't have to go through a 2ww unless there was something worth tranferring in. In answering August's question, Dr. Holland, if you have any other input to add, it'd be appreciated...

A.N.

I think that mother nature may decide for me. 6 of 7 fertilized, but two "look a little bit granular".
I'm wishing for 4 good ones so I don't have to make any decisions about whether I feel ok about transferring 6...

Hello Dr. Hollands,

Was just wondering what your thoughts were on Two day Vs. Three day transfer of embryos.

Day 2 and 3 can produce good results, it really all depemds on the clinic.

Is it not better to wait for the three day?

In terms of embryo selection possibly but then again others argue that it is better to get the embryos back to the uterus as soon as possible

What are the chances of achieveing pregancy with a two day blast?

Blastocysts take 4-5 days(sometimes 6 days) to form so a day 2 blastocyst isn't possible. Many clinics have excellent results with blastocyst transfer.

Good luck!

Peter

I look forward to hearing from you

Thank you,
August

Well, when push came to shove, my RE apparently no longer was too happy to put 6 in either...maybe they'd have had to be poorer quality for him to proceed. 4 went in yesterday, and 2 went for vitrification....
