Very low Fertilization rate with ICSI

We had out ER done on thursday sept 24. They retrieved 13 eggs, and 10 of which were mature to use. Out of the 10 eggs, only 4 fertilized with ICSI. Then when we had our ET we were down to 3 embryos. When we did our transfer, our RE told us that our embryos are "below average". One was 4 cell, one was 5 cell, and one was 6 cell. We had all 3 tranferred. I am very disappointed and shocked that our fertilization rate was so low. Why would the fertilization rate be so low with ICSI? Ours was 40%, and normal is 70-80%. I hate not knowing why.

hi hockey,

I also have low fertilization rate with ICSI. My eggs, before ICSI, were abnormal and/or wonky - problematic. My doctor suggested doing a modified cycle next time where they don't push my body so hard to produce so many eggs - just a few good quality ones. How were your eggs before the ICSI? Just curious....JEN

Hey Jen,

I don't know how my egg quality was. Neither the RE or embrylogist said anything to me about my eggs. When I asked my RE before my transfer, all he said is lets wait and see if these ones work. Then he told us to pray! So i don't know much of anything. DH and i were doing some research online last night, and found that even with poor sperm, the fertilization rate is 70-80%, and if the fertilization rate is low, is probably due to egg quality. I do have PCOS and I've read on this board particularly, that women with PCOS tend to have poor quality eggs. So right now, I am in the 2ww...trying to keep positive.

How was your fertilization rate?

Hi hockey,

My fertilization rate was also low. I have a few traits of PCOS, but not enough to give me the diagnosis (I get semi-regular periods, etc). Anyways, I've had 2 IVF's and never had any frosties (the embryos would simply stop). Also, my rate of fertilization was low, even with ICSI (especially after the 2nd IVF). In the first, we had 9 mature eggs and 6 were fertilized, we transferred 3 (they were noted to be top quality by the way!!!) and the other 3 didn't grow to blast. In the 2nd however, we had 8 mature eggs, but only 3 fertilized - very low! Everyone said I'd get pregnant with these embryos when we did the transfer - they were A++ quality and my lining was thick and juicy.

It was only the 2nd IVF that we looked at the lab reports of my eggs BEFORE ICSI. The notes are anecdotal, it's not as accurate as sperm grading, DNA fragmentation tests, and embryo grading. But the notes were the same in both cases - eggs show abnormalities. It's really ****ed me up - sorry for swearing. I thought my eggs were fine because my AMH is low/moderate 17.5 (so I have some eggs left) and my FSH is below 10.

My RE is saying that maybe pushing quantity has made the quality bad. So we may do a modified IVF cycle and try to produce 3-4 eggs, hoping for better quality. I'm also on Metformin (just started a couple of days ago) which is said to possible improve egg quality).

Sigh. I've just pretty worn out now and am looking to adoption. Not sure if I can afford donor egg or if i even want to do that.....

Interesting that you post this, as I've been having the same questions swim around in my brain. My first cycle we got 14 mature eggs and 10 fertilzed with ICSI. 7 went on to become blasts. Two transferred, one took, but I miscarried.

In my second cycle, there were only 7 mature eggs and only THREE of them fertilized. We transferred all three on day three because we didn't want to risk it. BFN.

I've got PCOS and we have very mild male factor, hence the ICSI. So now I wonder - wtf is wrong with my eggs? Do we even have a shot at them ever fertilizing naturally, given how badly they did when fertilization is forced?

I feel your pain! I'm going for my review appointment on Monday and will ask to see any notes from the embryologist regarding my eggs.

October 3- PIO working. Progesterone increases from 8 to 74, and beta of 27.
October 7 -(original beta date) 151 . Low positive.
October 9 - 273. Not doubling but still 'within standards'
October 14 - 1482. 97% doubling rate! Good news!
October 20 - u/s not great. Baby measuring a week behind, no hb.
October 27 - wonky again. Measuring 6w1d and I'm 7w4d. But there's a h/b of 89.
November 3 - negative growth, but hb of 114.
November 10 - miscarriage, 9w4d

September 21 - negative beta. Epic fail.

Apparently I make crap eggs. Taking 800mgs/day of Co-Q10 since October 2009, then will do high dose IUI sometime in June/July.

Will likely start up the blog again http://seedsininfert...il.blogspot.com when we get rolling.

Please, please, please...

i'm SO getting sick of this. And at this point, needing a counsellor. would you do donor egg??? i just am at my wit's end. what if I do donor egg and then don't get pregnant?? then be $20,000 in the hole??? we are by no means wealthy. we're solidly middle class. what to do????

excuse my language. but i'm just so so lost. i imagine you feel also frustrated....

#4 Fresh IVF with IVIg, exploring immune testing - start BCP Sept 20. ER around week of Oct. 25
9 eggs retrieved, 1 immature, 1 fractured. 7 to work with. 4 out of 7 fertilized (Day 3: 1 8 cell grade 1, 2 8 cell grade 2 and 1 morula). Day 5 transferred 1 great blast and 1 morula (1 morula and 1 10 cell did not make it to freezing).
Beta Nov. 11 - BFP! (550) I am in shock!
Issues with spotting, Nov 20, second beta 11,000 and early ultrasound: saw gestational sac and yolk sac in the right place but concern about a cyst or possible ectopic near my ovary as well, 4 more ultrasounds over a 10 day period, last ultrasound on Dec. 2 'cyst' has not grown and baby measuring on target at 7wks, 1day with a heart beat of 153bpm.
Now taking Lovenox due to 2 heterozygous thrombophilias
Ultrasound on Dec. 18 (9 weeks, 5 days) baby on track, hb 167
Ultrasound on Dec. 22 due to extra heavy spotting, baby still measuring on track, hb 167 and dancing
Ultrasound on Jan. 8 (12 weeks, 5 days) baby on track, hb 160, baby doing crunches!
18 week anatomy scan scheduled for Feb. 16 - they lost my blood work for the second part of the IPS so no results for that today (they took blood again) but the 18 week scan went great, baby looked awesome and even gave us a little wave in between sucking her thumb and inspecting her toes. Yes, I said "her" - it's a little girl!
20 week ultrasound at Mt. Sinai and they agree, so far so good.
IPS results are in and they are excellent!
EDD: July 19, 2010
Baby girl arrived at 39 weeks, 2 days! July 14, 2010 3:12 am, weighing 7 pounds even.
May 2013 - The IVF Journey continues...one last try to see if a sibling is in the cards...
Did LIT, IVIg, Lovenox and low dose aspirin. 5 eggs retrieved, 1 immature, 1 fractured, 3 to work with, 2 fertilized with ICSI.
June 5, day 3 transfer of 2 8 cell grade 2 embryos.BFN
April 2014, one very last try - natural IVF with intralipids this time, 2! dominant follicles, 2 mature eggs retrieved, both fertilize with ICSI, day 3 transfer scheduled for April 13, transferred 1 8-cell grade one and 1 6-cell grade one (also using Lovenox and low dose ASA) -BFN, we are really done, not sure if it is now my immune system, or old eggs now too.

We also did ICSI with a lower than normal fertilization rate. We had 15 mature eggs and only 7 fertilized. We put back two and of the remaining 5 only 2 made it to blast. Our clinic likes to see a 50% fertilization rate. Good news is my embies & blasts that made it were top quality.

The average of fertilization from ICSI is around 70-80%. However, there is individual variation and cycle difference as well. Likely, the low fertilization after ICSI is due to egg problem. In order to fertilize an egg by ICSI, egg has to be activated first. Failure of this process can lead to no fertilization although sperm was injected into an egg. Also, there may be sperm problem. If the sperm has round-head, called globozoospermia. It can also cause low fertilization or no fertilization (it`s not common). In your case, I feel that you may try again, perhaps, next cycle may be different.

This is an old thread, but I just wanted to add that we had low fertilization rate with ICSI and still had a positive pregnancy result. So if there is anyone out there experiencing this, don't lose hope.

In our case they retrieved 21 eggs, only 9 were mature and only 4 fertilized using ICSI (which is a 45% fertilization rate). I was very upset at the time, but in the end it didn't matter. I am now at 9 weeks with a little bean.

HI everyone,

I just wanted to add that my husband and I went through the samething, 13 retrieved, 12 were good, 8 fertilized and then we were down to 4 on the day of the transfer and not 1 of them was at the stage it should have been. We've been told that the chances of having a baby with my husbands sperm is not very good. We recently found out that my husband has a balanced translocation, a defect with his chromosomes. You could get tested for this if you haven't already, it's called the karyotype testing.
