
What is your FET med protocol with Hashimoto/ Endometriosis

I had 2 chemical pregnancies last year (April and June) and went through a lot of bloodwork to figure out what had caused them. I already knew I had endometriosis and hypothyroidism, but the blood tests revealed high thyroid antibodies so it means it is Hashimotos causing my hypothyroidism. I also had high glucose levels (started in August, but used to be normal).
Now trying to rule out a displaced window of implantation with ERA testing, which should happen in 2 weeks. I asked my fertility doctor what hed change in my med protocol, but apart from adding Lupron and keeping me in hyperthyroidy, he doesnt want to change anything for next FET. I know that Hashimoto affects the implantation and I dont understand why he doesnt want to do anything about that.
I want to trust him, but we only have 2 normal embryos left and our finances wont allow us to start over with creating new embryos. Id like to compare my numbers and protocols with some ladies in a similar situation to see if I should go to a different fertility doctor for a different protocol...
So my numbers:
TSH: 0,03
TPO (anti-thyroid antibodies) 696 IU/ml
Glucose: between 6,5 mmol/L and 8,4 mmo/L
Hormone levels were normal before, during and after FET protocol
Med protocol:
Folic Acid (5mg/day), baby aspirin (1/day), Estrace (estrogen, 3x2mg/ day), Crinone (morning), Progesterone in oil (evening), Dexamethasone, Vibramycine, Fragmin, Lupron.
Anyone has Metformin and/or Prednisone in their protocol?

January 2019: good news, 8 x 4aa (5 days) embryos created.
PGS test: 4x DNA champions, 4 abnormal
First ever FET on April 18th, 2019 -> 1st [email protected]= 7, 2nd [email protected] = 2.2... shooting star

Second FET on June 25th, 2019 ->1st [email protected]= 36, 2nd [email protected] = 2.3... another shooting star

Several tests to try explain poor implantation (CD-138, Mutations, Antibodies...) all negative except for TPO
February 2020: ERA test, I am 12 hours post-receptive!
March 2020: FET cancelled due to Covid

Third FET on June 29th 2020> complications during transfer, BFN. Left with only one good PGS tested embryo

Hi Apfel,
I'm sorry to hear about your chemical pregnancies. i hope this cycle is successful for you.
I have a similar diagnosis. Your protocol looks similar and more fulsome than mine. I think you have pretty much everything covered.
Lupron can treat endometriosis. I'll be on 3 months of Lupron depot shots (once per month) with add-back therapy before my FET.
Fragmin is an anti-coagulant. Along with Aspirin, it's thought to help with clotting issues. I'll be on Fragmin this time. It sounded like my doctor said it might have some sort of autoimmune properties as well?
Dexamethasone is a corticosteroid, like prednisone. Some doctors think this will help with the thyroid antibodies, although my doctor won't prescribe it for me.
Vibramycine is an antibiotic. It's not standard at my clinic but it is at lots of places.
The rest of your meds are standard for a medicated FET cycle.
Metformin is usually used for diabetes or people with PCOS. If you have high glucose levels, it might be worth asking your doctor about it. I take Inositol, from a naturopath, which apparently controls blood insulin levels, so it might be something else to look into.
Keeping your TSH low sounds like a good idea.
So I think overall it sounds like a good protocol. (I'm not a doctor or an expert though!) Hopefully it will make a difference for you!

For some reason, I didn‘t see your post rightaway in February, sorry! :‘(
I am getting ready for my next FET on Monday (March was cancelled due to Covid), I now feel confident that my protocol covers everything (doc has removed Metformin which has very nasty side effects, I now control my blood sugar levels with a monitor and have been normal the whole time (under 6). I have been taking Prograf and Plaquenil for two weeks, had an intralipid infusion on Tuesday, then will start Prednisone on transfer day. I am hopeful that this will keep my immune system calm and allow for a good implantation.
How is it going on your side?

Hi Apfel! Glad to hear you got more testing and your protocol seems to cover everything. I'm following you in the cycle buddies thread, I have my fingers crossed that things will go well for you!
