Surgery for Stage 4 Endo?

I have bilateral endometriomas with suspected DIE and I'm currently trying to get pregnant. Tried iui, and ivf. Still waiting for a positive. I've had 3 miscarriages, 2 of which were from trying naturally. Does surgery for Endo improve implantation in stage 4 Endo? If I'm asymptomatic, would surgery still be recommended? I'm worried surgery may actually start to cause pain that I didn't have before and want to avoid it if possible.

I would say if trying IVF and ovaries are easy to access for egg retrieval, surgery is unlikely to help your chances of implantation or risk of miscarriage with embryo transfer. However it may have a benefit on the chances of spontaneous pregnancy or with IUI (although there is a chance it would not help either).
