Hello Dr.

I am feeling very frustrated as my FET cycle using a GS was just cancelled. The embryo is from a 5x proven donor.  I was her 6th donation in a shared cycle.  The other half got a baby and I got a twin miscarriage.  We are now using frozen blasts from that cycle and moved on to a surrogate.

 

The GS has 3 children of her own with normal pregnancies. She is 49 years old, extremely healthy and has a regular period every 25-28 days.  

 

Last month, she took estrace for about 10 days and then added progesterone suppositories (and a pill) for five days.  After she started the progesterone, she started spotting.  We were told not to worry but to stop taking baby aspirin. We did and proceeded to transfer.  Result negative.  

 

This month, the exact same thing happened, (spotting starting on the 2nd day of progesterone) and on the day of transfer, it was cancelled with a recommendation of a sono before trying again.  Her lining was 12 B which I know is good.  Of course I am resorting to Dr. Google and see lots of people proceeding succesfully with spotting and also some cycles getting canceled.  What is your take?  Thank you.

p.s. on both cycles, she also took medrol, doxycycline, baby asa.  The second cycle, she added prednisone and  did intralipids (what a waste).

BREAK of 6 months turned to 8: took accupuncture, lost 30 pounds, added exercise, turned 41, Day 3 FSH down from average 7 to 4, worked on marriage relationship strained from dealing with IF
05/09 IVF #3 Gonal F and menopur. 5 Follicles but none mature. Cycle canceled.
07/09 IVF #4 Known DE cycle, 10 follicles, 1 sad egg, no fertilization. WT*?
08/09 Started taking DHEA (why didn't I know of this before?)
01/10 IVF #5: Convert monitoring cycle to natural cycle IVF due to 2 mature follicles being observed on ultrasound
01/10 1 grade 1, 8 cell embryo transferred. BFN
04/10 Estrogen Priming Cycle was cancelled for some reason I forget. known DE planned
08/10 IVF #6 Estrogen Priming Cycle with massive doses of FSH = nothing = cycle canceled and told to go away.

12/10 Known DE backs out
02/11 IVF #7. 2 embryos, both transferred. HPT+ 8dp3dt!!!, Beta #1 14dp3dt 278, Beta #2 16dp3dt 604, Beta #3 18dp3dt 1158 (not quite doubled...), first ultrasound--> TWINS!! March 25 only one slow heartbeat. April 2 no growth, slow heartbeat, April 6 no heartbeats--miscarriage at 9 weeks. D&C.
7/11 FET #1 BFN
9/11 IVF #8 Sad looking embryos. 4 transferred on day 3. BFN. Advised to move onto surrogacy. d e v a s t a t e d and b r o k e
04/13 Adopted 2 blastocycsts: BFN. Sperm banked before DH treated for cancer..
11/13 Very last attempt. New clinic. DE. Add intralipids and viagra. 9 eggs, 5 mature, 3 fertilized, 1 made it to day 3. no surprise, BFN..
04/14 Second "very last attempt" with 4th DE IVF.
04/05/14 13 eggs eggs retrieved, 7 mature, 7 make it to blast, 2 transferred, 2 frosties, BFP!! First Beta 4/21/14 = 92; Second Beta 4/23/14 = 265; 3rd Beta 4/25 613. May 9 ultrasound, right on track 6 weeks 5 days and hb 113 bpm. .
05/13/14 bleeding. May 14 no heartbeat. May 16 D & C. Are we allowed to say F words on here?

Sounds like this is quite a long and thorough history. 

Many RE's prefer that there not be bleeding at the time of transfer as it may reduce the chance of success of an embryo transfer. However the evidence is not clear to prove that.

It may not be a bad idea to consider changing the type of progesterone, or to review this with your RE for other ideas.

I could be wrong as different clinics have different protocols but the protocol I had was to take estrace for 18 days before starting the progesterone.  Same with my mock cycle - 18 days of estrogen before starting progesterone.  And that was only after an ultrasound to check thickness of lining which checked out fine and then advised to go ahead start progesterone.


1993 - started ttc, Aug 97-1st IVF BFN, Nov 97 FET BFN,  Aug 98 - 2nd IVF BFN, Nov 98 FET BFN, 1999-2000 - several IUI's - all BFN's, 2002 - 3rd IVF BFN, FET BFN, 2004 - 4th IVF - cancelled for poor response, 5th IVF with known donor BFN

June 2006 - ***** BFP (naturally)******  2007 *****DS was born

2008 - TTC again, 2010- started peri-menopause, 2013 - full menopause , 2014 -exploring donor egg

Nov 2014 - transfer 2 5 day embryos  Dec 2014 - BFP ***** DS born*****

Sept 2017 - transfer 2 5 day embryos - BFP

10 days of estrace should be sufficient if an optimal thickness of the lining has been achieved, so Sweetheart I would not be oncerned about that part.

thank you for the replies!  My GS says she understood the doctor to be concerned about the viability of her uterus and whether there may be polyps.  I don't understand that because she had a lot of testing in January.  She did not have a sono since she had one in 2014.  Her last baby was around 2009.

 

If she is not getting enough progesterone, would that cause bleeding?  I ask because the doctor very much preferred her to use PIO but because she is afraid of needles, we used suppositories 4x/day plus prometrium tablet daily.   Thoughts?

 

Thank you very much.  I thought when I switched to DE, I would have a baby and when that didn't work, I thought switching to a surrogate was the answer. It's been 11 years!

If the doctor thinks there is polyps that certainly could be a reason for irregular bleeding, and that should be assessed and polyps should be removed prior to transfer. No question.

 

If polyps are the issue then I think the method of progesterone is unlikely to have contributed to the spotting. The truth is in most of the literature there is no difference between PIO and suppositories in terms of pregnancy rate. However some clinics anecdotally find improved pregnancy rates with one method over another.
