Hello Dr.!

My name is Barbara and I would like to get some answers for my questions.
I have done 3 IVF with ISCI too of them ended with pregnancy but not for long. I have lost both of my pregnancies at 8 weeks. I run some test for recurent pregnancy loss and I was told to do some blood test, hysteroscopy and more. I also, had once polip removed. Now they are telling me that is possibility that I can have polips or many cyst which are preventing the pregnancy to grow. My question is If I have so many cysts which are preventing pregnancy from growing can they be removed with the guaranty that they will not come back. Can I go after that with IVF again or I can not. I'm very worrry becasue I'm 42 and my time runs, runs fust. What are the chances that I get prenant again.

Thank you in advance for your help Barbara :

Dear Barbara,

The tests for recurrent pregnancy loss often look at the following issues for women:

1. ovarian reserve
2. hormone balance
3. karyotype (genetic screen)
4. coagulation screen
5. autoimmune/alloimmune screen
6. anatomic screen

It would be #6, the anatomic screen, during which intrauterine polyps can be detected. Polyps are most easily characterized by 3D sonohysterogram, but a 2D sono is fine, and sometime a regular transvaginal ultrasound will pick them up. (Hysteroscopy is the gold standard of course, but it is a surgical procedure and thus used only when necessary).

Polyps are not usually dangerous for your health, just like skin tags are not usually dangerous. But when a skin tag is in the wrong place it can be very irritating; when an intrauterine polyp is in the wrong place (at the fundus of the cavity) it can prevent implantation.

If your clinical team has found polyps, they may suggest hysteroscopy to remove them. There is no guarantee they won't come back I'm afraid. Same for ovarian cysts: even if you remove one, another can arrive.

But all of this is treatable.

Barbara the answers really lie with your ovarian reserve: at age 42y, some women still have good chances, but many do not. A low ovarian reserve is *not* really treatable, so this is the key issue for you.

My advice is to talk to your doctor to get a realistic assesment of your ovarian reserve. If they still think that you have enough good eggs, then it can still happen for you.

Yours,

TGH
