Next IVF with PCOS and recurrent miscarriages?

I did my IVF#1 Jan/Feb and FET#1 June. Both ended with miscarriages. My local OB/GYN ordered karyotype, Antiphospholipid syndrome (APS), among other tests of clotting disorders. Karyotype tests have not come back, other tests came back normal. 

 

Just saw our RE on Friday (we are from out of province). If karyotype comes back normal, other than putting me on Metformin for PCO, he does not plan to do anything different. It will be the same antagonist protocol, even the same dosage of Gonal-F. He does not consider testing other possible immune issues and does not believe that anything can improve egg quality... If our genetics come back abnormally, the PGS will be the FISH method. I hear that CGH is more accurate...

 

We were contemplating on switching to VFC that takes immune issues seriously, but not really sure that's the best move. Switching clinics may take longer to do IVF#2 and current clinic is relatively more convenient for us. 

 

We would highly appreciate you ladies' opinions.

3 clomid cycles. Not respond to clomid.

 

IVF#1:

Nov 2013: offered and accepted IVF BCP+antagonist protocol

Jan 16, 2014: started injections of gonal-f and luveris

Jan 20, 2014: started Cetrotide

Jan 24, 2014: coasting

Jan 25, 2014: HCG trigger shot

Jan 27, 2014: ER, 11 eggs, 9 mature

7 fertilized

Feb 1, 2014: ET one day 5 blast

Feb 2, 2014: two frozen day 6 blasts

Feb 14, 2014: official BFP

Mar 4, 2014 (about 7 weeks): Started to bleed, increased cramping 

Mar 7, 2014: Cramps became unbearable, even with prescription pain killers. Called ambulance and went to E.R. D&C next day. zjujfjf.gif

 

FET#1:

Apr 30, 2014: started BCP

May 19, 2014: started suprefact

June 3, 2014: started estrace

June 19, 2014: started endometrin

June 23, 2014: transferred 2 day 6 blasts

July 6, 2014: official BFP

July 8, 2014, beta decreasing

July 10, 2014, beta dropped below 25. chemical icon_cry.gif

July 16, 2014, ob/gyn ordered tests for recurrent pregnancy loss

 

Aug 15, 2014, consult with RE. Starting metformin

Sept 16, 2014, karyotypes come back normal, anticardiolipin also normal

Sept 18, 2014, finally reached RE after two days of calling clinic. OK for IVF #2.

 

Nov 3, 2014, AF came and called clinic

Nov 4, 2014, Offered and accepted IVF #2.

Dec 1, 2014, AF came & started injections

Dec 14, 2014, ER, 14 eggs retrieved, 12 mature

11 fertilized

Dec 19, 2014, ET 2 day-5 blasts, 4 day-5 blasts frozen

Dec 20, 2014, 2 day-6 blasts frozen

Dec 25, 2014, ER visit for increased heartbeats of 120/min. ER doc couldn't find a reason why, thought I was dehydrated. Found out my HCG level was 93 smile.png

Dec 29, 2014, admitted to hospital for severe OHSS

Jan 10, 2015, finally discharged from hospital. Drained 5.5L of fluid in total. One good thing about being admitted to hospital was they checked my HCG levels several time and did quite a few U/S so that I knew the progress of pregnancy

Jan 19, 2015, 7w U/S, two sacs, one heartbeat 141, measured at 7w

Hi,

I'm so sorry for your losses. Have you been on metformin before? I also have pcos, this is the first drug my RE gave me. I felt so good on metformin. I lost a significant amount of weight and had some menstrual regularity return.

I also had a successful IVF cycle. There is a lot of literature on met improving egg quality and miscarriage rate (I was kept on it until I was 12 weeks pregnant).

I feel it was a big part of my success. I think in your case I would consider at least a second opinion.

Aug. 2012 - DX with PCOS (started metformin)

Aug. 2012 - Oct 2012 - clomid + trigger , timed X2 - BFN

Oct 2012-Jan 2013 - clomid + trigger IUI X3 - BFN

 

Took a break until the spring to begin IVF -

 

June 1 - Started BCPs

June 24 - Start stimming....75iu meopur, 75iu puregon

June 27- (moved up to 100iu of puregon)

June 29 (moved up to 150 iu of puregon)

July 1 - scan...6-10 follicles expected on ER day

July 3 - last day of stimming, HCG trigger

July 5 - ER.....8 Retrived, 5 mature, 5 fertilized

July 7 - 5 embryos

July 8 - ET (2, grade 1 embryos transferred), 3 embryos to be frozen (1 - grade 1, 2 - grade 2)

 

July 22 - BFP!!! 

July 24 - Beta repeated - numbers doubling....fingers crossed for a healthy pregnancy!!

Thank you very much for your response and congrat on your BFP!

 

I have never taken metformin. My RE never mentioned it before we saw him last Friday. I have never had regular menstrual cycles, but I am not overweight (BMI=21.7) and no other symptoms of PCOS

 

How long have you been on metformin before you did IVF?

I was on metformin a 10 months before my IVF cycle. I was using it while I was trying other fertility treatments (timed intercourse, IUI), which ultimately didn't work for me.

I have friends who have 'thin' PCOS whi have also been put on metformin for egg quality and miscarriage prevention. It may be worth trying. I found very few side effects. I'm going back on it when I stop breastfeeding.
