Should I do a Lap to test for Endo before IVF?

Hi everyone,

 

I have asked this question in other forums, but I was told that I may get more info if I asked here too!

 

I am currently unexplained. Have one daughter aged 3, then had one miscarriage, and nothing since....that was 1.5 years ago.

 

I am currently on my 3rd injectible IUI. 

 

I have a few relalives with severe Endo, so I am wondering if I should have a Lap to confirm this before moving on to IVF?  My doctor said it wasn't necessary as it wouldn't change the IVF protocol?

It was protocol here for me to get an Lap even though I have no history of Endo in my family and we knew DH had issues.  Not sure what the reason is... but thought I'd share with you how it is here in Qc.


May 2013 - We welcome our baby boy

Hi,

 

I had a diagnostic lap last year (since the HSG could not confirm if my tubes were blocked) and I ended up having extensive endo.  Some RE's believe that having a lap to remove endo before IVF is necessary, others believe that IVF bypasses endo so it is not required.  Both my doctors recommended removing the endo before IVF so I had another surgery in April to remove it before starting IVF.  The problem is, you have to wait quite some time to get an endo surgeon who is willing to remove it, if there is endo.  So I guess it comes down to how quickly you want to get pregnant.  If you have endo and it is removed, you have a chance at getting pregnant naturally after the lap, since you have proven fertility.

Not sure if this is helpfiul or not. But what about just having an operative hysteroscopy instead? I had one done and found it so simple, no recovery. When I had mine done I signed the consent forms for a lap as well, so if they had found anything, they could do they lap while I was asleep..Do you have any Endo symptoms? Good luck!

Heregoesnothing- wow, routine? That's crazy that they make everyone do it.  I guess it is to rule out everything that they can though.

 

RMad- That is what I am having such problems with....like everyone else I want to be pregnant as of yesterday! BUT, I am also concerend about my probable endo and how that will effect things.  Maybe getting the surgery done will help me get pregnant! Sigh...

 

Emily81- Can they tell if you may have endo from the hysteroscopy? I guess I am not very familiar with what each surgery does. I do want them to have a look inside my uterus just incase there is something there that was missed with the HSG.  I do have Endo symptoms, but all my life I was told it was nothing, or that is was all IBS symptoms.  I have had countless ultrasounds and even a colonoscopy where they found nothing, but still told me it was my bowels? 

 

This is so frustrating.  My doctor tells me that finding endo will make no difference to my IVF treatment, but everything I read says that it should make a difference!

 

I also have a very strong history of severe endo in my family.

Oh, I was also informed that it would be my RE doing the Lap if I so choosed to do it.  Does that mean if he finds something that he will just look around and not do any surgery, close me up and then I would have to have another one done by another surgeon?

Hi,

That's how it worked for me. I was told there are only a few skilled surgeons that remove endo, but perhaps it is different for your doc. That's definitely a good question to ask your RE.

Hi I would highly recommend a lap before ivf....for 3 years dr couldn't figure out why I was not getting pregnant, had all the test and bloodwork that came back all normal...anyways ive been in the unexplained category... but now after my lap last week I now have a diagnosis of stage 2 endo just found out today my obgyn was able to remove the endo and scarring. Part of me is sad but another part of me can move on to ivf and know that whatever protocol they put me on will be specifically endo "related". I really had to push for the lap with my obgyn as they weren't concerned with my symptoms (painful BM during period, spotting CD9-CD14 every month, painful menstrual cramps) After consult with my RE he suggested a lap and my obgyn finally agreed. You really have to advocate for yourself. I'm glad that I know what I have now before starting ivf. good luck on your journey.

Dec 16- BCP superfact,  Dexamethosone 

Jan 12-Jan 22- menopur 225iu

Jan 25--> ER 22 eggs retrieved 17 fertilized ivf/icsi split,,,, E2-22000 risk of OHSS no embryo transfer

Jan 30--> 5 x day 5 embryos to freeze (cavitating blastocysts 2x19 and 3x17+)

Plan for MAY FET!

 

FET # 1-->May 12 - Transfer of early blastocyst grade 18

                 May 23-  BFP!!!!!!!! Beta 1- 159

                 May 26- Beta 2 - 686

                 May 28- Beta 3 - 1517

So it sounds like if it is mild/moderate endo then they just remove what they can then and there.  If it is more severe then they have to call a more specialised surgeon in to treat it?

Baby-Bun - Yes, it is free in Quebec - not sure how it worked with everyone else.  It might depend on your Dr. as well and what they prescribe b/c I know different clinics work in different ways.  But, I had to have it done which I found bizarre since we knew we have MF.  They saw nothing when they did it.  Then about 2 years later, while doing a routine u/s to start an IVF cycle, they suspected a polyp.  I then had to go back and do a hysteroscopy this time to remove it.  Had I had a polyp when doing the lap, they would have seen it and removed it.  So, it's certainly not a bad thing to have it done.  A polyp can prevent implantation as well...

Hi :)

 

I'm also in QC and all the REs I've seen stated that it was not necessary to remove it since IVF would bypass the endo. I think it's standard in some clinics but not all of them. Since I bought what they told me, I went ahead and tried IVF without having the endo removed... well, long story short, I'm now on a quest to find a Gyn that will remove it....  Personnaly, I think that if it can increase your chances, or remove any doubt, go for it... you may wait a few months but it could be beneficial. You may have endo with NO symptoms whats so ever. I was told by an RE (in Ontario) that endo acts like a little army.. kills everything in it's path... sperm, embryo etc, raises hormone levels can have an impact on egg quality and AFC.. Better be safe than sorry...

And may has well make it a trio...male factor (motility)

 

After 2 failed IUIs, 5 Failed IVF:

Trying on our own until spring come's along... 

 

Feb 09: A little Miracle, a natural BFP. Praying it sticks around for 9 months.

Feb 11: Beta 25 (yikes)- P4: 47 oh boy, put on progesterone

Feb 13: Beta 86 - P4: 144

Feb 18: Beta 544 - P4: 96

Feb 20: Beta 1128 - P4: 81... increased progesterone to 600mg

Feb 24: Beta 1608 - P4: 100... oh boy, not looking promissing

Feb 25: Beta: 1942 (no P4) at another hospital.. ok, looking better... U/S: sac and yolk. Nothing else.

March 04: HB 115. Problem: Sac is small... repeat u/s next week

March 14: U/S , HB (abdominal) 167bpm (internal) 158bpm. Problem: Sac still small, reapeat u/s in 10 days..

March 24: U/S confirmed no HB.. stop progesterone. M/C that same evening

 ivf may bypass the endo, however the endo can impact implantation especially through inflammation or toxins it can excrete as well. Im glad I did it before we spent all our savings for ivf. At least I know I have it now

Thanks girls. I think I am leaning towards the surgery. I am terrified of everything! Terrified of the surgery, what the surgery will find or won't find, of IVF, of egg retrieval... Everything!!! My head feels like it's spinning. At least with the surgery I can better plan in my head what to do next it know that I have endo or not.

I had a lap done in Aug 2012 and cycled Oct/Nov 2012 and got pregnant with twins who are now 4 months old. If they think it could be the cause it couldn't hurt. I see you're at One Kitchener. I saw Dr. Cockwell at One Oakville, she's now on her own at One Mississauga. She specializes in endo, so maybe getting a referral to her would be a good idea. She lasered mine while in there and then success!
Me: 33 with PCOS and Endometriosis
DH: 43 with male factor resulting from vasectomy/vasectomy reversal, poor sperm count following reversal
July 2010 - DH's Vasectomy Reversal, whole sperm on one side, heads on the other, hematoma formed on left testicle, took 3 months to completely recover
Sept 2010 - First RE visit
Sept 2010 - HSG, Saline Sono, routine BW all normal
Oct 2010 - First SA total count 8 million, 20% motility
Oct 2010 - Clomid cycle with BD, BFN
Nov 2010 - Clomid cycle with BD, BFN
Dec 2010 - Clinic closed over holidays, no treatment allowed
Jan 2011 - supposed to have first IUI, cyst on right ovary, E2 high, cycle cancelled, on BCP for 6 weeks for proper timing due to vacation

High FSH, Low AFC (only 6)
Endo (duuuh) & DOR  (great....)

And may has well make it a trio...male factor (motility)

 

After 2 failed IUIs, 5 Failed IVF:

Trying on our own until spring come's along... 

 

Feb 09: A little Miracle, a natural BFP. Praying it sticks around for 9 months.

Feb 11: Beta 25 (yikes)- P4: 47 oh boy, put on progesterone

Feb 13: Beta 86 - P4: 144

Feb 18: Beta 544 - P4: 96

Feb 20: Beta 1128 - P4: 81... increased progesterone to 600mg

Feb 24: Beta 1608 - P4: 100... oh boy, not looking promissing

Feb 25: Beta: 1942 (no P4) at another hospital.. ok, looking better... U/S: sac and yolk. Nothing else.

March 04: HB 115. Problem: Sac is small... repeat u/s next week

March 14: U/S , HB (abdominal) 167bpm (internal) 158bpm. Problem: Sac still small, reapeat u/s in 10 days..

March 24: U/S confirmed no HB.. stop progesterone. M/C that same evening

Had my appointment today. My Dr agreed that a lap would be a good idea to see what is going on in there. Doing a hysteroscopy as well. Just waiting on a date now, they said probably January / Feb. We also have our IVF info session next week. Is there any questions I should be asking about the surgery? I think my RE will be doing it.

I've never attended and IVF info session, so I don't really have any suggestions. I'm thinking that as the session progresses, questions will come to you :)

Everything just feels so surreal right now.  Like everyone on here, I never thought I would be needing to do all this.  I know I am still 'new' here, but it feels like its been forever.  How are things going with you? Are you making any progress with treatments?

I am also so scared of what they may find when they get in there!!  I don't know if they plan on removing anything if its extensive.  Ugh, this is so frustrating and too many things to think of. My doctor said if he finds a little bit he will remove it, otherwise I think they will leave it.  Not sure what to do about that!

It is surreal... no one wants to be here, but I'm happy that they are ;). I've actually read that a lot (more than they think) of unexplained IF case are actually asymptomatic endo... which can only be confirmed by going in. It's not always shown on an u/s. So maybe they found your cause and can fix it :D YAY for fixing people, loll.

 

I found a local ob/gyn that is taking the endo seriously. I gave him 18 tubes of blood, I have an u/s on Dec 05 and depending on the results, it's MRI time, loll. He will then make a decision on what to do. After he is done with me, he will send DH for a detailed S/A and more blood tests (since he only had the hiv/hepatitis test done). I have to admit that I am quite please and looking forward to whats going to happen next (even if it means a lap :D)

that is awesome that they are looking into all of that for you!  I am terrified at what path I will be taking.  I will be turing 35 in June, so kinda freaked out about that! I am totally hoping its just a quick fix, but thinking not likely.  I guess I just have to take one day at a time!  keep me posted on your test results, I am very curious!!

Hi 2nd chance :), My u/s went well, but I wont get any results (u/s and blood test) until next Thursday. I can't wait, lol. My original appt was on Jan 17 (when I called thats the earliest appt they had) but the doc wanted to see me 1 week after the u/s so I stopped to see the doc after my u/s and he squeezed me in between 2 patients for next week. Talk about service, LOLL.

 

And you??? Did you get a date yet?!?! I'm hoping it's going to be very soon :)
