No Sperm

Hello

So my DH and I went to get the results of his SA today. No sperm. In 2 samples. No sperm. HIs hormones levels are also low. The urologist has referred us to a fertility Dr in Hamilton.

The urologist wasnt very good at explaining what will come next. I did some research and it looks like my husband maybe looking at a biopsy.

My question is, has anyone's husband out there had this done and is it painful?. I just want to prepare my husband as best I can. And does anyone have a ball park figure of cost of ICSI in Southern Ontario? Either London or Hamilton (closest clinics to us).

If anyone has any answers, they would be greatly appreciated.

Jan.2012 Urologist explained that 1st SA was actually zero, ordered 2nd SA, which was zero as well.
Feb.23/12 Appt with reproductive urologist in Hamilton-Felt husband may have a blockage. More Bld work and another SA
April 19/12 Appt with Rep urologist-Results from SA = A FEW SPERM!! woo hoo!! Bld work reveals low testosterone. More blood work and possibly an MRI.
Aug 2012 Husband placed on Arimidex 1mg/daily x 3 months in hopes to increase sperm count.
Aug 2012 Met with HART fertility clinic.
Oct.13 2012 Results from SA = No sperm. Fertility doc placed me on clomid, although I did not respond. He says I am still ovulating but my follicles are small.
Oct. 2012 Chose and ordered donor sperm. Now we wait until i ovulate.
Nov.9/12 Inseminated with donor sperm
Nov. 22/12 BFN
Now we wait until the end of January for the next cycle, due to clinic being closed throughout December for Xmas.

Hi there,

Not sure if this is the samething, but........

My Dh has a 'rare' sperm count.......almost nothing. He had a Tese operation done and yes, it was most painful. He was off work for a week.

The cost of the operation (back in 2007) was $1300 and not covered by OHIP.

We cycle in London, LHSC, The Fertility Clinic. The cost of ICSI $1500. You can check out their website for additional costs.

U/S - Aug 3 - 35 weeks - induction date decided!! thank goodness - 'cause I am done!!!!!!!!!!!!!!!!!!! Induction date is August 18th - but I am already 2-3cm dialated - so we'll see if I meet 'the date'. Baby A weighs 6 lb 6 oz and baby B is 5lb 15 oz.
U/S - Aug 13 - 37 weeks - still sitting at 3cm dilated - confirmed induction date with L&D August 18th.

This pregnancy has been a long haul and I'm glad that it's nearly over!

Our 2 healthy baby boys arrived on Sunday August 19th - we are thrilled and over the moon......family is finally complete and now we can move on to the next chapter in our lives.......

Hi Kitten,

My DH also had no sperm in his samples. The Urologist did a bilateral testicular biopsy and found he had no sperm producing cells, a very rare condition known as "Sertoli only syndrome". He was under anesthesia for the biopsy and felt pain for a couple of weeks after the surgery, it is painful but manageable with pain meds. Unfortunately a biopsy is the only way they can really determine what the diagnosis is. Sorry you're going through this, I know what a heartbreaking ordeal it is. Hi Kitten,
As I'm in the US we used the California Cryobank. Not sure what banks there are in Canada, your RE or Urologist would be able to give you more information.
DH and I looked at profiles together online and chose one who resembled DH both in character, ethnicity and physically. We were lucky to find someone we both felt happy about. It's a process. It was very hard and weird at first. We have an anonymous donor, although some people opt for an open one who is willing to be contacted by the child in the future. CCB offers detailed descriptions of donors, has them write an essay and in our case we were able to see a baby photograph of the donor.

Our clinic required counseling with a psychologist who had to evaluate us as a couple before we could proceed with using donor sperm. The counseling was very helpful for both of us as obviously this is a very hard decision to have to make and I'd definitely recommend you take your time before deciding upon that. At the moment you're still at the testing stage and I really hope they're able to find some sperm in your case. Donor sperm has to be a mutual decision. I was actually far more reluctant to consider it than DH and it was him that convinced me in the end that it was the right thing for us.

My husband just had a biopsy done a couple of weeks ago to determine if he had viable sperm. His SA came back saying no sperm so they did the biospy. We are in Alberta and he was not put under just a little sedated. They did give him a prescription for pain medication but he never used it just a couple of advils a few times. He was surprised the how tolerable it was. He biopsy was on a friday morning and he was back at work monday morning......even shopping for a couple of hours on sunday morning! Good luck to you. We just got our results back last week and all his sperm was mature which of course we were over the moon happy about as the urologist prepared us to believe they would not be mature.

We are currently waiting on the urologist appointment, but i wanted to ask what you are doing now? Now that you have gotten such wonderful new what are the next steps you are taking? Did the urologist make the referral? Will you be going straight to IVF with the ICSI(not sure if this is the proper accrynym option?

Hi Kitten,

I'm kinda in the same boat as you. My husband is going for an ultrasound next week to see if there is a blockage. A blockage would actually be great b/c they can extract the sperm. If he's just not producing sperm, we have decided to go with a donor. It was definitely a harder decision for me than him. Another suggestion my Dr. gave me about chosing a donor, is to have the blood type match. In our instance, we would like to never have to tell our child and this way if he ever needs a transfusion or anything, my husband is still a match. Depending on what you guys want to do, you might consider this.

My husband just had a biopsy done a couple of weeks ago to determine if he had viable sperm. His SA came back saying no sperm so they did the biospy. We are in Alberta and he was not put under just a little sedated. They did give him a prescription for pain medication but he never used it just a couple of advils a few times. He was surprised the how tolerable it was. He biopsy was on a friday morning and he was back at work monday morning......even shopping for a couple of hours on sunday morning! Good luck to you. We just got our results back last week and all his sperm was mature which of course we were over the moon happy about as the urologist prepared us to believe they would not be mature.

We are currently waiting on the urologist appointment, but i wanted to ask what you are doing now? Now that you have gotten such wonderful new what are the next steps you are taking? Did the urologist make the referral? Will you be going straight to IVF with the ICSI(not sure if this is the proper accrynym option?

We are going to calgary (from Edmonton) March 8th and 9th for the IVF orientation. I am not sure which procedure we will be doing.....I assume the ICSI as that seems to be what others are saying. We had the refferal made to the Foothills clinic from my fertility dr here in edmonton.... Dr. Sagel. We actually had the orientation booked in before we even had the biopsy done. It took us less than a month to get the urlogist to do the biopsy. Dr. Macalum ( I think) She is leduc. The most wonderful urologist and dr. She was so thorough at explaining and got my DH 2 weeks later for his biopsy. We then go the results back another 2 weeks later. Would have been sooner however there was xmas closure. Now we just sit and wait for March. Hopefully you do not have to wait long for your urologist appt.

Me: 34 (35 for 3rd try - 36 when daughters were born)
DH: 32 (33 -34)

We are seeing Dr. Macalum on April 4th. It was a 6 month wait for us to get into her so now its the waiting game. Can i ask why you chose Calgary and not Edmonton fertility clinic? Was it a quicker wait time?

We didn't have a choice. Edmonton does not do IVF so we used edmonton for the testing to determine the cause and then they referred us to Calgary when they new it was IVF we required. You will enjoy Dr Macalum. She was so wonderful and never rushed the appt. She aslo works with the calgary fertility clinic which is helpful. We got rushed in as my DH has only one testical and they wanted to get an MRI done to make sure nothing was seriously wrong. 6 months....yikes. It's so hard to be patient in these situations. I wish there was a magic wand with a quick fix.

Strange this is the one I went to for the testing and the one that refferred us to Edmonton. They said they do not have and RE and only do IUI there and not IVF???? HHHMMMM

That is really strange. Do you live in Edmonton? Was the DR you went to just an OBGYN who had an interest in fertility? I hope that being referred to Calgary hasn't put you out too much...

Wow, i just called my husband in tears thinking this was such a huge set back. My Dr referred us to the Edmonton clinic in November and i am just waiting for the call for my consult appointment. Thought i had to go get a new referral to Calgary! Thanks for the clarification. Not that I can guarentee I will being going through IVF as I dont even know if my hubby has swimmers blocked somewhere until April. Thats the other part I am having such a difficult time with! I feel like if i knew i could atleast start planning, whether donor IUI or hubby IVF! Waiting game sucks! And i have no idea how so many people get through all of this. I never thought before I found out that soooo many couples have to go through this. If it wasnt for these forums I would be completely lost.

We just got the same diagnosis on Dec 1 and see our RE a week from today. He has had no further testing other than his two SAs, so we have no idea what to expect. There is an azoospermia thread here that I have been reading too.

I read a lot about donor sperm, but my husband isn't ready for that until we know if he has sperm or not. We're both on a rollercoaster of emotions, going from feelign really hopeful to downright terrified of how much it will cost, the stats, and pain.

Becky (35)  Unexpectedly widowed in Feb 2013.  The night after my ER.  I was married to an incredible man named Chris.

Son, age 6
TTC #2 (his first) since our wedding in July 2011.

Dec 1, 2011- Found out he has azoospermia (no sperm in SA.)  Totally heartbroken and shocked.
Jan 2012- First consult at fertility clinic.
March 2012- My tests show high LH but everything else was normal. His LH/FSH were high, testosterone was really low. (4.2)
May 17, 2012- Official Dx of testicular failure and non-obstructive azoo. Given the option of mTESE, TESA or using a donor.
June 7, 2012- Urologist is seeing us again per our request. We need him to answer some questions about the procedures and testosterone.  He would not consider Clomid or any hormones for DH.
June 27, 2012- Saw RE and decided to proceed with dIUI. Told we needed one cycle on the BCP because of my high LH.
June 28, 2012- Started BCP for 21 days. Did pre-IUI micro and blood testing on July 3rd. Told they should be in on time for IUI in early August.
July 21, 2012- CD1. Called clinic and found out my cycle was canceled. They messed up and my file wasn't ready. They forgot to draw vit D and some other results are missing.
August 9, 2012- Nurse left message saying they cannot do this cycle either. Now it will take 2-3 MONTHS for DH's blood tests before we can use donor sperm.
August 10, 2012- RE called. DH has a reactive Hepatitis C blood test and they are trying to figure out if he's got an active infection or was exposed in the past.
August 23, 2012- He's Hep C NEGATIVE! Phew!
October 2012- Latest SA results still shows azoo, but we're glad the testosterone looks normal now. dIUI after 21 days on the pill. Clomid for cd3-7, LH is too high (17 and 14.6) and progestone getting too high. Had 3, 13.5mm folliciles and my lining is 8.6. CD13- Lining was good, triple layer at about 9. Now I just have one 17.5mm follicle on my left side.
October 17, 2012- dIUI  canceled. Clomid didn't work, estrogen too low, LH was normal again and progesterone was off too. Now we need an appointment to talk to our RE again to start over.
Dec 2012- Saw RE and we have to do IVF/ICSI, even with a donor. So, we decided to throw in a TESA with donor backup. Bloodwork this cycle, BCP and mock in Jan and hopefully a Feb IVF/ICSI/TESA!
 
IVF #1
Mock done, teaching visit done.  Started Suprefact on Jan 30.  AF arrived on schedule.
Baseline done, starting Gonal-F and Luveris Feb 11.
Stim day 4-  1 follicle at 11mm, 40 more under 10.  My lining is already a triple lining and at 8.6
Stim day 6-  8-9 follices between 11-13mm at least 20 more under 10mm.  This doctor saw my lining at 6.3?   Encouraged to walk because my circulation to my uterus wasn't what they wanted to be this time.  No idea what that means.
Stim day 8-   14 follicles.  All measuring between 11 and 15 with more under 10.   Lining is over 10.  Circulation is good again.   They are letting me skip my day 9 appointment. 
Stim day 10-  TRIGGER tonight!   I have 14 between 15-21mm, a few more at 14 that may catch up.  Lining is over 10.   DH's TESA is tomorrow.
Feb 21-  No sperm found with TESA.   ER tomorrow with donor sperm.
Feb 22-  ER was successful!  Collected 18 eggs.   7 from my right and 8 on my left.  We can skip ICSI because the donor's sperm is great.  Crossing fingers for a good fert report tomorrow.
June 2013- Updating. My husband, Chris, passed away unexpectedly of Sudden Arrhythmia Death Syndrome just hours after my ER on Feb 23.  Had 14 fertilize.  9, 5-day frozen blasts are at the clinic waiting for me to make some decisions. 

 

FET #1

June 2014-  5-day blast.   BFN just in time for what would have been our 3rd anniversary.  

 

FET#2

August 2014.  FET #2 on Aug 25 with another 5-day blast.  BFP!   Had scare with a bleed at 4w5d, but beta is still doubling.  Due May 13, 2015.  Ultrasound on Oct 5.

Oct 5-  8w4d-  Blighted Ovum.  Stopping estrogen and progesterone.  Waiting to miscarry.  Looking forward to moving on with life.

Oct 17- D&C.   Feeling peace about being at the end of the infertility battle.  Looking forward to the next chapter in my life.

That is really strange. Do you live in Edmonton? Was the DR you went to just an OBGYN who had an interest in fertility? I hope that being referred to Calgary hasn't put you out too much...

She is one of the Fertility dr's at the Edmonton clinic. I called them this afternoon and they said they do not do IVF in edmonton only IUI. It's a bit much having to go to calgary but I am from there so I at least have my family and friends there for support when we go. It will just be hard with taking all the time off of work. But alas I will do what I have to do!!

What was the name of the DR who told you that?

I am thinking that maybe what they are telling me is because we have a sperm issue and that's why we have to go to calgary? I so very confused why we were told they do not do IVF there but in fact there are ladies on this site having IVF done there. Posted Image

Maybe they don't do the TESE procedure there to extract the sperm? Or the ICSI you'll need?

Becky (35)  Unexpectedly widowed in Feb 2013.  The night after my ER.  I was married to an incredible man named Chris.

Son, age 6
TTC #2 (his first) since our wedding in July 2011.

Dec 1, 2011- Found out he has azoospermia (no sperm in SA.)  Totally heartbroken and shocked.
Jan 2012- First consult at fertility clinic.
March 2012- My tests show high LH but everything else was normal. His LH/FSH were high, testosterone was really low. (4.2)
May 17, 2012- Official Dx of testicular failure and non-obstructive azoo. Given the option of mTESE, TESA or using a donor.
June 7, 2012- Urologist is seeing us again per our request. We need him to answer some questions about the procedures and testosterone.  He would not consider Clomid or any hormones for DH.
June 27, 2012- Saw RE and decided to proceed with dIUI. Told we needed one cycle on the BCP because of my high LH.
June 28, 2012- Started BCP for 21 days. Did pre-IUI micro and blood testing on July 3rd. Told they should be in on time for IUI in early August.
July 21, 2012- CD1. Called clinic and found out my cycle was canceled. They messed up and my file wasn't ready. They forgot to draw vit D and some other results are missing.
August 9, 2012- Nurse left message saying they cannot do this cycle either. Now it will take 2-3 MONTHS for DH's blood tests before we can use donor sperm.
August 10, 2012- RE called. DH has a reactive Hepatitis C blood test and they are trying to figure out if he's got an active infection or was exposed in the past.
August 23, 2012- He's Hep C NEGATIVE! Phew!
October 2012- Latest SA results still shows azoo, but we're glad the testosterone looks normal now. dIUI after 21 days on the pill. Clomid for cd3-7, LH is too high (17 and 14.6) and progestone getting too high. Had 3, 13.5mm folliciles and my lining is 8.6. CD13- Lining was good, triple layer at about 9. Now I just have one 17.5mm follicle on my left side.
October 17, 2012- dIUI  canceled. Clomid didn't work, estrogen too low, LH was normal again and progesterone was off too. Now we need an appointment to talk to our RE again to start over.

I had a few people now tell me they are doing ICSI there? lol This is strange. I have a call into the clinic to explain this to me.

Hey kitten keep me posted,  My husband has little to no sperm from two samples and we have been referred to the urologist in mississauga we don't have the apt date yet but love to know what to expect.
