I had a bad cold...all in my chest three days before I pPROM'd. I was still suffering from this cold when I did pprom. I think what happened with me, the infection started down below...infected the sac, then hit the placenta which enters in the bloodstream of baby and mommy. Mommy gets a nasty cold and so does the baby. In the autopsy report, Karolina essentially died of pneumonia...which is sorta what my cold was like.

You know looking back...I asked does a cold like this seem normal while pregnant? And the doctors said yea its normal. Well...its not...obviously.

Yea, I will be going in with every symptom as well.

So, Valasia, will you be doing a cervical clerclage or will they be doing a we will see how it goes and do one if they find it necessary?

It's done by doing lap. Not too sure on the healing time but I don't think it's too long. It's only a quick surgery and they send you home after. The surgeon is calling me early next week to discuss what's happenning, whether he's coming here or if I'm going there. Hopefully early in the new year...I'll post when I know.
I went for a u/s with the triplets and my cervix was 3.2cm and two days later my membranes were bulging. With IC it can happen whenever and with no labor symptoms at all. So, that sucks!

I had a lot of discharge before my loss. I also had pain in my cervix area. The problem is that increased discharge is a normal symptom of pregnancy as well so even if you tell you doctor they will tell you that it is normal. It's not until you have a loss that they will start looking at as a symptom of infection or IC.

The unfortunate thing about internal scans is that if you are suspected of having an IC or have had chorioamnionitis, they actually need to do internal exams and swabs to make sure it doesn't happen again.......or at least that is what my high risk OB says.

The thing that I'm struggling with most is that my doctor doesn't think my issue is mechanical......she doesn't think it's an issue of my cervix being strong enough. She things the chorioamnionitis caused my cervix to open. Therefore she doesn't think a preventative cerclage is appropriate at this point. Part of me just wants to get one just to be safe but I don't want to make the decision for myself because cerclages have risks of their own to the baby. AHHHH!!!!

Sweetest.....we have made the decision to go for a cervical cerclage. The peri wants to do the watch and wait with weekly scans. I've had lots of time to think about this and for us the risk of having the cerclage placed preventatively outweigh the risk of watch and wait especially when IC can happen so quickly.....to quickly to be caught. It would be terrible to lose another baby in either scenario but there is def a difference between having an early loss D&C and having a baby die in your ams. I know I would feel guilty either way....a m/c resulting from a cerclage or a pre term delivery because I didn't trust my gut and get the cerclage. No easy answers in all this.

Stellabella....I know you said your dr was a top dr in all this....but I still have trouble understanding how they can definitely say the chorio caused cervical dilation especially since they can't tell us how the chorio got there in the first place and the most common method of getting it is through ascending infection. I had acute chorio it wasn't yet in the cord, I felt fine, and only had slightly elevated neutrophils....and that was when I was already dilated.....so where did it come from? You'd think if I had something in my bloodstream some sort of infection that got to the placenta I would have had some symptoms.

Valasia - I echo your sentiment.....there are no easy answers in all of this and that's why it's so frustrating. My doctor said that the infection could have been there before the pregnancy and was asymptomatic. They can be caused by invasive procedures in your uterus. I had a D&C 4 months before I got pregnant.....I think that if the infection was there before it was from that but who knows. I'm trying to trust my doctor (I have problems trusting doctors as I've been screwed so many times by them) and I really like her....but part of me also worries if the D&C might have caused an IC. They are also reluctant to diagnosis me with IC as I've had a full term pregnancy before. It's kind of a chicken before the egg scenario with IC and infections.
The more I think about it though the more I think I am going to push for a cerclage. I've had most a first trimester loss at 11 weeks and my recent loss at 17 weeks and there was no comparison. It hurt losing at 11 weeks but I knew the baby wasn't healthy and that is why......but having a perfectly healthy die in your arms because of something in your body was the most heartbreaking experience I've ever been through.

Just out of curiosity for those of you trying again......does it change your protocol in terms of how many embryos to transfer? I spoke with my IVF doctor and think I'm only going to do a single embryo transfer with my next IVF.

Stellabella I have heard the theory of dormant infection. Before Gavin I would have had my c-section with B, hsg, and the iui that G was conceived through. Since Gavin I've had 2 sis, 2 hsg, 5 iui and a d & c. Would antibiotics taken at the time of ivf take care of any infection hiding in there?

I think in terms of our ivf a eSET is our most responsible option. There is a helpful presentation on the foothills clinic website that really helps to put that decision into perspective and honestly the only reason I would be compelled to choose a double would be monetary. I can only hope that if a eSET doesn't result in pregnancy/bring home baby that I might have some embryos frozen for a try at FET.

I'm not sure if the antibiotics with IVF would help with infections before. After we found out about the chorioamnionitis, my new OB did a biopsy to check to see if the infection was still there. We got the results back last week and it showed that there was presently no infection there right now. She said if there had been they would have treated me with antibiotics prior. I think are so many different types of antibiotics and they all treat different sorts of infections.

Just wondering if any of you were tested for V-Leiden or MTHFR (blood clotting disorders)? My OB sent me for the testing back in October and I just got the results that I have MTHFR. I did take low dose aspirin during IVF and my pregnancy - no idea at the time, but now I know.
I think that I might need to be on heparin for this during pregnancy. I have my pre-pregnancy with the high risk OB in February and I have a whole whack of questions to ask now.

Just thought I would see if any of you had this testing done.

Yup....I had the whole thrombophilia panel done because my baby's placenta had a clot on it (again they can't tell me how or why!). Anyways my whole panel came back fine and I even had some of the tests repeated with my last mc. We have added baby aspirin to the regime because it might help and won't hurt.

I hope getting these results gives you a little more confidence going into a sub pregnancy....at least there is something you can go on.

I see my RE tomorrow. Fx for karyotyping results and a plan :)

Valasia - how did your RE appointment go?

I got a huge surprise today - Mount Sinai called me and asked if I could meet with Dr.Whittle (high risk OB) next week. I've been waiting almost 5 months to get in to see her. I definitely took the chance, since they had me booked for February. At least now we can make a game plan for the next pregnancy and I can get all my questions answered. It's like I can get all these things sorted out in 2011 before I embark on a new year and leave this year behind me.

Well my appt went ok no karyotyping results should be in next week as they called in a rush (it's only been 6 months!). Anyways it seems my re was under the impression that I've had 5 losses which is not correct. I was able to get the RPL label off my chart and we are now good to go for ivf in January!

I should see af that first week of Jan. I need a sis and afc but those can be done while cycling....I am ready to start :)

SweatestDream - I'm so happy you got the appointment with Dr. Whittle! She is amazing. I feel so lucky to be with her as well.

Valasia - it looks like we will be cycling at the same. I'm expecting my next period around Dec 26th and starting birth control for 2 weeks before I start sims. I'm thinking more and more I'm going to get a preventative cerclage.....hopefully Dr. Whittle will go for it.

Glad to see you guys cycling soon:) That is exciting. I won't be ready until April/May, but having all my meds in the fridge keeps me very motivated.

So, what are you doing/taking prior to your cycles?

Are you guys doing Acupuncture?
What vitamins are you taking?

I spoke with my RE this week and she seems to think that Materna..or the drug store brand is okay for a prenatal. I was hoping she would put me on Pregvit5 already, but I guess not.

I am on preg vit 5, I also take metformin for my pcos and I will start baby aspirin again when we cycle. As for vitamins I take c, d and e. I also take inositol which is part of the b vitamin complex that really seems to help with my pcos symptoms. I've done acu in the past (ds1) but I don't really have the time for it now. I will focus on eating healthy, exercising and hopefully dropping a couple more lbs.

Stellabella.....glad to hear I will have a buddy :). Def leaning towards the cerclage.

My re recommended transferring 2 despite my history. Anyone have any thoughts about that in your own situations?

Why the pregvit? I know my RE had me on them but when I talked to the perinatal dr's they strongly advised me against that. Folic acid should be kept to 1mg/day.

Lol....Hoping gotta love the conflicting info! Dr M put me on preg vit right after losing Gavin, I though this was to help restore my body of vitamins and minerals lost during the pregnancy since we were set to ttc right away. When I ran out of refills I assumed they (our clinic) would tell me to do a otc prenatal but they didn't, apparently they are recommending it for everyone and gave me another years worth. When I was preggo this year I did see my ob the one time who said take if just for the first tri and after that a regular prenatal was fine.

I was also told to take the Pregvit5 until the end of the 1st trimester. And then after that I could switch a normal prenatal providing I was eating calcium rich food, or I could actually take calcium supplements as well.

Now, with this MTHFR thing...apparently I should be on a more organic form of folic acid so that it absorbs better because blood clotting disorders apparently do not take in the folic fully leading to higher chances of birth defects. Anyway..my RE did not mention this at all. I am hoping the high risk OB does next week. I have quite a lot of questions.

My RE wants to do a single embryo transfer. I am willing to do another double...and my DH is on the fence. We will see what happens. Last time we only ended up with two embryos, so it was worth it.

I have PCOS as well, so I am on metformin. I just went from 1000mg to 1500mg...and it sucks! How do you deal with the nausea, if you have any? What is your protocol for IVF? I dunno what exactly my RE has planned, but I know I will be going on BCP, suprefact, Puregon and ovidrel. She also opted out of the progesterone suppositores and prescribed Crinone, which is a gel in an applicator. I am not filling this prescription until later, but I want to talk to my OB about this. I sorta wanted to be on the Progesterone in Oil shots instead.

As for the stitch, it's still up in the air, again...will see what my OB says. I will do whatever it takes.

My DH and I have been talking about just not even bothering for me to go to work at all during IVF and pregnancy. I am currently off on STD and do not want to go back in February. I've had a hard time at my job, so we will see. It will mean looking for a job until IVF starts. Do any of you plan to put yourselves on modified bedrest for the duration of your pregnancy?

Sweetest I've been on metformin for years and I don't have any gastro type side effects anymore. I guess my body has adjusted to it. I am on 850 2x per day. My clinic does "natural" starts meaning no suppression. I asked about this because with pcos I read it's better to supress our higher lh levels before stimulating. The re said it wasn't necessary. I am on BC (NuvaRing) right now so hopefully those hormones are keeping my wacky ones in check. As for the rest he mentioned 300iu of fsh (likely puregon) once my lead follie gets to 14 they add the antagonist till trigger then prog supps after ER.
If we do get pregnant my plan is to have a cerclage placed around 13 or so weeks and at the same time be written off work till delivery I don't think my ob will deny this (I hope!) We have std plan at work so that is good. I will take it easy for sure couch/bed rest whatever it takes.

Hi,

I'm right now taking Coenzyme Q10 (600 mg), Inositol (I don't have PCOS but my doctor says it helps with overall egg health), fish oil, Vitamin E, Vitamin C, Iron (my reserves are on the low side) and Probiotics (to help protect against future infections).

I'm also trying to eat healthy and exercise. I had gained almost 15 lbs during my last pregnancy so I worked really hard at eating healthy and exercise to get the weight off for the past 4 months, so I'm feeling pretty healthy now! I started seeing a naturopath too a couple months ago that really helped. She was really against using Materna......she says that the vitamins are really badly absorbed. She put be on Prenatal SAP from NFH.....I have to take it 3 times a day but to be honest I see such a huge difference. I feel a lot better and don't get constipated like I did with Materna and Pregvit.

I'm only transferring one. We are actually going to clinic in Denver to do our next IVF so that they can genetically test the choromosomes of our embryos and only transfer a healthy ones. Statistically transferring one embryo that we know is genetically healthy gives us the same chances of concieving as transfering two untested ones. Our doctor in Toronto suggested going this route to me because of my history (always transferred more than one and only got pregnant with singletons) and because this will be my 6th IVF and I really want it to be my last......I don't think I can keep doing this......physically and financially. The treatment in Denver is really expensive (double what it is here) and we have scrapped together everything to do it......I'm really hoping in the end it is worth it.

I had my meeting with the High Risk OB for pre-pregnancy.
They want me to do some blood work for blood clotting disorders, a full work up.
And, they also want to check for bacteria and they mentioned I will probably be on antibiotics the entire pregnancy. They do not think it is a cervix issue. I am on the fence about this..but on a good note, they will be checking me weekly to see if anything changes.

Has anyone had these bacteria levels checked? They told me that it is checked at the vagina, cervix and uterus level. The uterus level is done by taken a tissue sample from the endo layer at week 3 of your cycle. If anyone has done this, how painful is this? Do I need someone to come with me for this procedure? I am planning on going alone, but if its painful then I might not want to drive all the way home by myself and will bring my DH.

Ohh...and not sure if I mentioned this already, but the high risk OB suggested a one embryo transfer. They also mentioned to me that the loss of the one twin early on would not have caused the pprom. They think that there are bacterias in our body that are good, but because out immunity lowers during pregnancy, these bacterias end up turning into an infection and causes PTL.

Sweetest....I havent had any dr mention an endometrial biopsy, I did have some vag swabs done when I was pregnant last and the plan was to repeat them at each appointment.

Gotta love that "they think" and not know the reasons. Nothing more frustrating IMO!

I had the biopsy done by Dr. Whittle. I found that it did hurt a little bit (but I'm super sensitive)......but nothing compared to some of things we go through in IVF. It doesn't take very long.....it's kind of like a PAP smear test but a little more "scrapping". The funny part is that I was totally fine afterwards and then she showed me the sample of blood and tissue and I got light headed and almost fainted (I'm a bit squeamish!) I think for most people it's not a big deal as it's a simple procedure and they wouldn't need someone to drive them but I was happy that my husband was there to drive me home. My results came back about 3 weeks later and luckily they were negative.

Thanks Stellabelle for the information. They did mention it would be fairly quick the procedure. I hate getting paps done. It is suck a yucky feeling..but if it has to be done it has to be done.

So, have they made a plan for your pregnancy? Did they discuss being on antibiotics for you for your entire pregnancy?

Yeah, we have a plan. She said that after my pregnancy is confirmed I would go see her between 8-10 weeks for my first appointment. I would then have weekly appointments with cervical checks and regular swabs for infections. She had a question mark about antibiotics and cerclage......I guess it would depend on how the pregnancy went. She also suggested that once I get pregnant to attend their support group for pregnant women who have suffered later loss. She also strongly suggested a single embryo transfer......she said that it wasn't just because of my previous loss but rather because as a high risk doctor she sees how risky multiples.
