IVF w/ICSI #1 ended in a miscarriage at 7.5 weeks (no heartbeat). Our RE thinks we have a sperm quality/fragmentation issue (I am 29, DH is 51). If we do IVF again, we are interested in a new technology I've read about "PICSI." My clinic doesn't use the technology so I would be traveling to a clinic that does PICIS. Needless to say, I am trying to gather as much informaiton on PICSI as possible before making such an important decision.

I have done extensive reading, so I understand the science of HBA binding. My question is whether you think this is a legitimate answer for couples with high sperm fragmentation. In other words, I have read that it is "controversial" but I don't see where the controversy is. From what I have read, it seems pretty straighforward.

Thanks for any information.
Me 29 - low progestrone, needed trigger shot to ovulate, had stage 2+ endometriosis removed
DH 51- poor sperm quality, low volume, (possibly) high dna fragmentation

TTC since Aug 2009

IUI #4 May 2010
Laproscopy for stage 2+ endo May 2010
IUI #5 July 2010
IUI #6 August 2010

IVF #1 November/December 2010
Stimming since 11/23
ER Dec 3 (20 eggs obtained, 15 mature)
12 eggs fertilized
Transferred two "excellent grade 1 blastocysts" on 12/9
No frosties
13dpo BFP!!! Beta 30
15dpo #2 Beta 58 (doubling time of 50.47 hours)
20dpo #3 Beta 679!!! (doubling time of 34 hours)
January 4, 2011 Ultrasound - Gestational sac, "baby" 4 mm ... no heartbeat.
January 11, 2011 Ultrasound. Miscarriage confirmed. :( We're devastated.

CONSIDERING IVF #2 for April/May 2011
RE believes DH's sperm have high DNA fragmentation (not detectible on standard semen analysis). Now researching "Physiologic ICSI" or "PICSI" for better sperm selection.

I have read a few studies and this and it doesn't seem like many places are offering this yet, I guess because it has not be proven effective yet. Sounds like it could be helpful to those of diagnosed with DNA frag. problems. I would be curious to hear some specialist's opinions on this too.
