Hi,

Is AH a common practice done on Blasts? I just recently had a Blast transfer and asked if they did AH on them, the RE told me they don't do it on Blasts because they don't need it. I have seen that some clinics do. Why the difference in practice?

Jan 2011 - FET 2 Blasts- BFN
lots of prayer, exercise, councelling and reflection- I am back to try again.
June 2011 - FET 3 Blasts
Beta is June 29th - POSITIVE...Holy Crap!
July 18- perfect 7w ultrasound
July 19- another m/c - my heart is broken.

Trying again in January with a new protocol. Good or bad this is our last cycle.
Jan 2012- IVF #4 Milder and longer stimulation phase - 2 PERFECT 1AB and 1AA fully expanded blasts transferred
Feb 6 Beta 274!!!!
Feb 8 Beta 577 - looking good!
Feb 10 Beta (just for fun!) -1160- Right on target!
Due Date October 12!

My clinic wont do it either. They said it would run the risk of making the embryo too fragile prior to transfer. I guess the walls are much thinnner at blast stage than day 3 and the chemical they use to 'thin' the walls would make things bad.
Dx: Me PCOS, DH Azoospermia TTC since 2003 1st ATTEMPT: CRC Feb 28/09 - ER & MicroTESE: 31 eggs retreived Next day - 21 eggs were mature & Donor ICSI'd - 17 fertilized!! March 5/09 - ET: 1 grade 4AA blast...7dp5dt AF arrived and brought with her OHSS BETA:BFN 2nd ATTEMPT: ONE Fertility August 15/09 - FET: 1x 4AA Blast BETA:BFN 3rd ATTEMPT: ONE Fertility Sept 18/09 - FET: Assisted Hatching on 1 Morula & 1 8cell compacting. BETA:BFN 4th ATTEMPT: ONE Fertility March 10 2010 - FET 2 blasts BETA:BFN 5th ATTEMPT: ONE Fertility April: 4 Serial Endometrial Biopsys. May 31 FET 2 Blast 4AB & 2CC. Mine only does AH on day 3 embryos not on blasts.

As for as I know, there is more than one way to do AH. Sometimes Clinics use a chemical, drop it next to it, and it erodes (for lack of a better word) part of the zona. Other clinics use a lazer ($$$)and actually take away part of the zona. From hwat I have 'researched' the lazer is more accurate and takes a larger part of the zona, therefor possibly decreasing chances of the embryo/blast 'splitting' in utero and avioding twins.

As far as AH on blasts, my Clinic does this (using lazer). It is quite common in the bigger more successful clinics in the States but not all Clinics do it.(I scoped the internet far and wide before opting for it). I am 42 and it was my first time using AH, it was also my first bfp on fet, but I am older ('mature' -ahem)the zona may be harder on my embryos/blasts than most. (Plus I am PCO).

I hope Dr.H comes back soon so that you get a professionl's answer :)This may tide you over in the meantime :)

Thanks for the replies ladies, I have booked a follow up with my RE to further investigate my case. My clinic uses the lazer to AH, but only on Day 3 frozen. My RE is quite conservative and may just not feel the literature supports it.
You are right Stacy, not many of the smaller clinics in Canada do it.

I have had NO success with Blast FETs, perhaps AH is what I need to push them a little further along before the ET.

The clinics I've been to did AH on blasts, especially if you are over 35 years of age due to a hardening of the zona.
