Selective reduction?

I never thought I would be in this boat, but I am. Reducing triplets to twins. How do people decide this? I've done so much research on it, I do believe now I have confused myself.

This cycle was our last attempt - this was a fresh cycle. We had 4 embies put back and I'm 41 years old. On fresh cycles, We've always put 3 back in and we were blessed back in 2008 with our DD. Our other attempts, none of the embies took. The quality of the embies for this cycle were good, but they weren't as good as when we got pregnant back in 2008 so I was never worried about twins or triplets. Before we had our ET the RE had said that she had never heard of a person my age getting pregnant with triplets, she said though there was a possiblity of twins. Dh and I were ok with twins, but we knew that any multiple birth had complications - but dh and I were convinced that we would only 'maybe' get one to stick out of the 4. So we insisted for the 4. Plus, I did not want to freeze one and have to do a FET for 1 embie - I couldn't do. Never had any luck with a FET and I had no desire to go back to the well if this cycle did not work, we were done completely and ok with being a family of 3.

Now here we are. We did this and now we have to deal with it. When I spoke to my case manager a few days ago at the clinic she told me that I had a 50% chance of losing the entire pregnancy if I reduced to twins - boy that was a shocker to hear!!!! I hadn't found that out on Dr. Google! I don't want to lose the pregnancy! Then I was told that London does not do selective reduction, that Dh & I would have to travel to Toronto for that and that Dr's wouldn't reduced triplets to twins......so I don't know what to believe! I just want the cold hard facts about this so we can make a honest decision and live with it!

If we didn't reduce, we have a home big enough for all 6 of us, so we wouldn't have to move. We would need the biggest van going - Dh has looked into SUV's but they don't carry 4 car seats at one time. We have found an Econo passenger type van that would carry 10-12 people plus storage. But what about the cost of college/university? And spending equal time with each child? So many thoughts.......

When I go for my u/s on Monday, I will be 9+ weeks - so I still have 'some' time to decide. Part of me is saying no, don't do it but the other part is saying do it, it would give the other babies a fighting chance. Does anyone know of a person in my age bracket that went for a selective reduction and what was the outcome for them?

U/S - Aug 3 - 35 weeks - induction date decided!! thank goodness - 'cause I am done!!!!!!!!!!!!!!!!!!! Induction date is August 18th - but I am already 2-3cm dialated - so we'll see if I meet 'the date'. Baby A weighs 6 lb 6 oz and baby B is 5lb 15 oz.
U/S - Aug 13 - 37 weeks - still sitting at 3cm dilated - confirmed induction date with L&D August 18th.

This pregnancy has been a long haul and I'm glad that it's nearly over!

Our 2 healthy baby boys arrived on Sunday August 19th - we are thrilled and over the moon......family is finally complete and now we can move on to the next chapter in our lives.......

Sorry you are facing these hard decisions
One fact at age 41 is that sadly, there is still a 40% chance of a natural miscarriage per embryo, so try not to stress too much yet about making decisions. As well, at your age, you may in fact need or want more info on the health of each embryo before you could consider reducing, but I'm not sure about the safety of earlier vs later selective reductions. I don't think there is an issue with reducing down to twins. The issue is that many places dont allow reductions down to a singleton. There is definitely one clinic in Toronto ( mount Sinai?) that does reductions, even down to a singleton and check my earlier post as while Dr Mark Evans is expensive his success rates are extremely high

Finances are not the only concern with multiples. They are definitely at much higher risk for lifelong issues like cerebral Palsy, etc

AN - i had looked at the links that you had posted. I went to Dr. Evan's website and did some more reading on him.

When googling Dh & i too found that Mount Sinai did reductions.

I didn't know that there was a 40% per embryo of miscarriage, that's kind of scary, but it would solve my situation (I don't mean for that to sound unwanting of the pregnancy, not the case at all). Yes, alot of info that I have read is the birth defects of having triplets to soon and to be honest, I'm not sure if that's something that Dh & I can handle. I had thought about doing the CVS test - from what i've read that test can be done fairly early 1st trimester...... that would for sure give us some info on the health of the babies and maybe be a deciding factor? I think i could ramble on here but honestly, I don't know what to do......this is all i think about these days.........

I just wanted to send you hugs, your in a hard spot, I know my husband and I spoke at length about reductions, and did decide in advance to reduce I case of twins as the survival rates of triplets were just so low.

Personally I would find the surgeon with the highest survival rate in Canada and go see them, no matter where they are experience is so important.

Be gentle with yourself.

Hi Lea,

I would at least see about a phone consultation with Dr. Evans. CVS does allow for earlier testing, but has higher risk for miscarriage than amnio has.

I know that it's not fun thinking about natural miscarriage, but sometimes nature does make its own decisions. My friend was 38 with IVF triplets and they spontaneously reduced to twins. Another woman in the over 40 thread was pregnant with triplets, that self reduced down to a singleton, but I don't recall at how many weeks.

I do suggest that you get all the information that you need to make an informed decision, and then take things one day at a time until you approach the time that you really need to decide.

Lea so sorry you're having to face this. Ther was another member who went through a reduction last year reducing quads to twins, strawberry, maybe try pm'ing her and see if she could share her experience with you. Hugs.
Me (Kerri-Lyn) 41, DH (Steve) 37, TTC since April 2007Unexplained IF (possibly crappy eggs, AMH 10.4 = poor responder) - HSG, SHGx3, Lap, Cycle Monitoring, Recurrent loss bloods, EMB, Karyotyping, DNA Frag - all normal! 7 IUI's, 2 cancelled IVF's, 3 complete IVF's, 1 FET, 2 clinics, 1 early miscarriage, lots of debt, lots of heartache, lots of tears. U/S - July 13 - 32 weeks - baby A has turned (head down) their weights - baby A 3lbs 15 oz and baby B 4lbs 3oz

U/S - July 27 - 34 weeks - part 2 - just getting a 'quick' u/s done - babies lookin' good...though, baby B is being squished by baby A.....
U/S - Aug 3 - 35 weeks - induction date decided!! thank goodness - 'cause I am done!!!!!!!!!!!!!!!!!!! Induction date is August 18th - but I am already 2-3cm dialated - so we'll see if I meet 'the date'. Baby A weighs 6 lb 6 oz and baby B is 5lb 15 oz.
U/S - Aug 13 - 37 weeks - still sitting at 3cm dilated - confirmed induction date with L&D August 18th.

This pregnancy has been a long haul and I'm glad that it's nearly over!

Our 2 healthy baby boys arrived on Sunday August 19th - we are thrilled and over the moon......family is finally complete and now we can move on to the next chapter in our lives.......

Lea - my friend put back four and all four stuck. She reduced to twins at Mount Sinai in Toronto. It was really tough on her but her twins are doing really well. They are thriving. Sorry you are going through this. It is okay to put in 4 at your age. Try to let go of any guilt because it is unwarranted. I don't know if mount Sinai will reduce triplets to twins but why not call them and ask directly? My two cents - first, congrats on your cycle working - in the over 40 category, that in itself is a feat. I know you need to think about the reduction possibility, but as A.N. said, the chance for miscarrying naturally before 12 or so weeks is very high.

If you still have all 3 going strong after the 13th week, and you want to consider using someone in the States, PM me -

Good luck - I would have put back 4 as well (at your age - or at my age!).

Faithlove - thanks for your thoughts and the referral form. I have spoken to another member on here who had quads and reduced to twins and she gave me the name of the Dr. that she used at Sunnybrook Hospital in Toronto. When I was at the clinic this morning and we were speaking about SR they told me that Sunnybrook would be my best bet - so I think I will stick with them - but again, thanks!!!

Sharlene, thanks for your kind words too! I agree getting pregnant over 40 is a feat in it's self - but as I have said, I never thought in a million years that i would have to decide this. I did put a call into Dr. Mark Evans (New York City) but I have not heard back from them......Now that I've thought about things I think I will stick with a Dr. in Canada as OHIP will cover the procedure where I was checking on line the prices of a SR and they were from 5-8K. That hurts the wallet!

I had another u/s today and all babies are doing very well - measuring on track and h/b are strong. They even had their buds for arms & legs.....after seeing that it makes the SR even harder to decide.......

Lea: I've never been pregnant so I cant imagine what it must be like. This is a very difficult decision that you have to make and no matter what you decide its the right decision for you and no one can ever tell you otherwise. I wish you the best of luck no matter what you decide. Hugs

Sep/10 - Dec/10 multiple tests, HSG, sperm alaysis, blood work etc
Jan/11: Dx with MF infertity and DH referred for micro-tese
June/11: 0 sperm found with micro-tese
made decision to use donor sperm
made decisions re: donor
6 IUI's between July/11 and April/12 - all BFN's + 2 cancelled cycles

IVF:
May/12 patch protocol however cycle cancelled as clinic mis-timed drugs
June/12 patch protocol cycle cancelled due to crazy hormones
Waiting on CD1 (again) to now start suprafact IVF cycle - no AF, stupid hormones

July/12 hormones are normal again. RE suggests puregon straight away with no suprafact. Confirmed ER 28/July - my head is spinning from all these changes!
ET: 02/Aug - 1 embie tranferred
Early Beta 09/Aug - BFP 66 !!!!! Unfortunately also have OHSS but at this point I dont care so happy
Beta 13/Aug - 326

Just wanted to share an up-date with you all.......

I saw the high risk OB today and had another u/s. We learned that i misscarried one of the babies. There was no heart beat. The other 2 babies are doing spot on. I was sad that I had lost one of the babies, but I also had a sense of relief that I did not have to make the decision to have a selective reduction. Sad to say, I was more relieved that I didn't have to make this decision than I was sad of the loss. I'm sorry if that sounds bad to anyone, but that last few weeks Dh and I have been all over the place with emotion - it's been so hard.

I just wanted to thank you all for your opinions and well wishes!

I'm sorry for the loss of the one, but it's a lot easier for nature to take its course rather than you having to choose to reduce and select one that could have been healthy. That is the last decision anyone wants to make and especially when it took so much to get here. I hope the rest of your pregnancy goes uneventfully!

I saw the high risk OB today and had another u/s. We learned that i misscarried one of the babies. There was no heart beat. The other 2 babies are doing spot on. I was sad that I had lost one of the babies, but I also had a sense of relief that I did not have to make the decision to have a selective reduction. Sad to say, I was more relieved that I didn't have to make this decision than I was sad of the loss. I'm sorry if that sounds bad to anyone, but that last few weeks Dh and I have been all over the place with emotion - it's been so hard.

I just wanted to thank you all for your opinions and well wishes!

I hope my post isn't taken badly - I'm really happy for you that things worked out as they did at this point. I feared that you would make a "selection" and then one would naturally drop out of the race. This way really is for the best - the one that didn't make it, probably just simply wasn't chromosomally perfect - the others will probably do just fine now - and so will you! You'll still have your hands quite full with twins - but you'll be able to manage with likely healthier children. Best wishes for a healthy and uneventful pregnancy and delivery of your twins!

Mouse, Frostedlemon & sharlene - Thanks for your kind words. My Dh wanted the SR done and I couldn't wrap my head around it - so I'm not sure where this would have taken 'us'. Like you all said, this was the best possible outcome for this situation.

U/S - Aug 3 - 35 weeks - induction date decided!! thank goodness - 'cause I am done!!!!!!!!!!!!!!!!!!! Induction date is August 18th - but I am already 2-3cm dialated - so we'll see if I meet 'the date'. Baby A weighs 6 lb 6 oz and baby B is 5lb 15 oz.
U/S - Aug 13 - 37 weeks - still sitting at 3cm dilated - confirmed induction date with L&D August 18th.

This pregnancy has been a long haul and I'm glad that it's nearly over!

Our 2 healthy baby boys arrived on Sunday August 19th - we are thrilled and over the moon......family is finally complete and now we can move on to the next chapter in our lives.......

So glad to hear that nature made the decision for you but sorry for all the emotions you've gone through. Wishing you a healthy pregnancy and no more turmoil:)

TTC from March 2009-March 2011 before DB had arrived on the scene

2 pregnancies with 7 week miscarriage requiring D&C
2 chemical pregnancies


April 2011 began a relationship now with a wonderful guy with an 8 year old son.
Sept 2011 Natural BFP while trying not to conceive (how ironic)
First Beta Day 15 from LMP was 851, Beta 4, 19 days post ovulation 5382
7wk US showed a heartbeat of 150! 9 wk US HB 185

May 19 2012 DS is born!!

Lea I know this is a old thread but you mentioned you had the name of a doctor at sunnybrook hospital in toronto that does selective reduction. I was wondering if you still had this name and can pass it on to me. I am in the process of calling doctors at sunnybrook and hitting roadblocks in my quest to find a doctor who performs this service in Toronto.
